摘  要:目的:探讨全膝关节置换术中髌骨置换的临床效果。方法选择2008年1月~2013年1月期间在我院骨科及广州珠江医院骨科行全膝关节置换术(TKA)的膝关节骨性关节炎(OA)78例患者,并按照数字表法随机分为两组,观察组39例实施髌骨置换的治疗;对照组39例则不进行髌骨置换,并比较两组的临床效果。结果观察组术后2、3个月的KSS临床评分与对照组比较差异均无统计学意义(P>0.05);而术后6、12个月KSS临床评分与对照组比较差异有统计学意义(P<0.05),观察组术后2、3、6个月的Feller髌骨评分与对照组比较(P>0.05);而术后12个月Feller髌骨评分与对照组比较差异有统计学意义(P<0.05)。结论人工全膝关节置换术中行髌骨置换能减轻疼痛且降低术后膝前痛的发生率,能够改善膝关节功能评分和髌骨评分,增加患者的满意度,疗效确切。Objective To investigate the clinical effect of patellar replacement in total knee arthroplasty. Methods 78 patients with knee osteoarthritis (OA) treated in department of orthopedics of our hospital and Guangzhou Zhujiang Hospital from January 2008 to January 2013 who received total knee arthroplasty (TKA) were selected, and randomly divided into two groups in accordance with the number table, 39 cases in the observation group implemented the treatment of patella replacement; 39 cases in the control group didn't receive patellar replacement. Results The difference of KSS clinical score postoperative 2,3 months between the observation group and the control group were not significantly different (P 〉 0.05); while KSS clinical score postoperative 6,12 months in the observation group was compared with the control group , the difference was significant (P〈0.05) , postoperative 2,3,6 months Fel-ler patella score in the observation group was compared with the control group (P〉 0.05); and postoperative 12 months Feller patellar score was compared with the control group, the difference was significant (P 〈 0.05). Conclusion Total knee arthroplasty intraoperative patellar replacement can relieve paln and reduce the incidence of postoperative anterior knee paln, can improve knee function score and patella score. To increase satisfaction of patient, curative effect.
