TOSIGHT视频喉镜与McCoy喉镜在气管插管中的应用效果

摘　　要：目的探讨TOSIGHT视频喉镜与McCoy喉镜在气管插管中的应用效果。方法 132例气管插管全身麻醉下择期行手术患者随机分为T组和M组各66例,T组采用TOSIGHT视频喉镜进行气管插管,M组采用McCoy喉镜进行气管插管,观察2组Cormack-Lehane分级以及插管总时间、插管一次成功率,记录血流动力学变化。结果 （1）T组插管总时间[（18.2±4.1）s]低于M组[（21.3±8.2）s]（P〈0.01）,且T组及M组Cormack-Lehane分级为Ⅰ~Ⅱ级患者插管总时间[（16.2±2.1）、（17.5±1.6）s]均低于Ⅲ~Ⅳ级患者[（24.7±6.3）、（33.0±13.5）s]（P〈0.01）;（2）CormackLehane分级Ⅰ~Ⅱ级患者,M组一次插管成功率（94.7%）与T组（100.0%）比较差异无统计学意义（P〉0.05）;Cormack-Lehane分级Ⅲ~Ⅳ级患者,T组一次插管成功率（81.3%）高于M组（71.4%）（P〈0.05）;（3）对Cormacklehane分级Ⅲ~Ⅳ级患者,插管即刻及插管后1min,T组收缩压[（92±10）、（103±9）mm Hg]、舒张压[（65±9）、（73±10）mm Hg]及心率[（87±9）、（85±8）次/min]均低于M组[（97±10）、（114±8）mm Hg,（57±7）、（69±11）mm Hg,（93±10）、（91±8）次/min],差异有统计学意义（P〈0.05或P〈0.01）;（3）T组并发症发生率（3.03%）低于M组（12.12%）（P〈0.05）。结论与McCoy喉镜比较,TOSIGHT视频喉镜在困难气道患者的麻醉管理中具有优势。Objective To explore the effect of TOSIGHT video laryngoscope and McCoy laryngoscope in tracheal intubation.Methods A total of 132 patients receiving tracheal intubation were randomly divided into T group receiving TOSIGHT video laryngoscope and M group receiving McCoy laryngoscope,with 66 patients in each group.The following data were observed including the Cormack-Lehane grade,overall intubation time,success rate of one-time intubation,and the hemodynamic alteration in two groups.Results The overall intubation time was（18.2±4.1）s in T group,significantly lower than that in M group（（21.3±8.2）s）（P0.01）.The overall intubation time of patients withⅠ~ⅡCormack-Lehane（（16.2±2.1）,（17.5±1.6）s）were lower than those in patients with Ⅲ ~ Ⅳ Cormack-Lehane（（24.7±6.3）,（33.0±13.5）s）in T and M groups（P0.01）.There was no significant difference in one-time intubation success rate in patients withⅠ~Ⅱ Cormack-Lehane between M group（94.7%）and T group（100.0%）（P0.05）.The one-time intubation success rate in patients with Ⅲ~Ⅳ Cormack-Lehane was higher in T group（81.3%）than that in M group（71.4%）（P0.05）.The systolic blood pressure（SBP）（（92±10）,（103±9）mm Hg）,diastolic blood pressure（DBP）（（65±9）,（73±10）mm Hg）,and heart rate（HR）（（87±9）,（85±8）beats/min）at intubation and 1min after intubation in patients withⅢ~Ⅳ Cormack-Lehane in T group,significantly lower than those in M group（SBP：（97±10）,（114±8）mm Hg;DBP：（57±7）,（69±11）mm Hg,HR：（93±10）,（91±8）beats/min）（P0.05,P0.01）.The incidence of complications was lower in T group（3.03%）than that in M group（12.12%）（P0.05）.Conclusion TOSIGHT video laryngoscope is superior to McCoy laryngoscope for patients with difficult airway.
