摘 要:目的:探讨全髋关节置换(THR)术后的前后姿势平衡与下肢肌力的相关性。方法:选取25例全髋关节置换术后的患者纳入实验组,25例健康人纳入对照组,分别进行平衡仪姿势平衡测试,通过软件得到压力中心相关参数结果。对实验组采用等速肌力测试仪测试手术侧和非手术侧髋关节屈伸和膝关节屈伸的峰力矩(PT)指标。并分析前后姿势平衡与下肢肌力的相关性。结果:1睁眼状态实验组较对照组前后标准差、横向标准差、运动椭圆面积差异有显著性意义(P<0.05)。闭眼状态实验组较对照组前后标准差和运动椭圆面积有显著的差异(P<0.05),对照组优于实验组。2患侧较健侧髋、膝关节的屈曲和伸展峰力矩差异有显著性意义(P<0.05)。3姿势平衡前后标准差与髋患健屈曲肌力比,膝患健伸展肌力比及患侧髋、膝屈伸肌力呈负相关(r=-0.446—0.652),且有显著性差异(P<0.05)。4患侧屈髋肌力是预测前后姿势平衡标准差的主要肌力因素。结论:全髋关节置换术后患者前后姿势平衡功能和屈髋肌力关系密切。Objective: To investigate the relationship between postural balance in the sagittal plane and the lower extremity muscle strength in patients with unilateral total hip replacement (THR). Method: Twenty-five patients with THR (trial group) and 25 healthy volunteers (control group) were recruited. Pro-kine line balance feedback training equipment was used to assess postural balance of patients and healthy controls. Isokinetic strength of hip and knee flexor and extensor of both lower extremities in patients was eval- uated with Isomed 2000 isokinetic dynamometer. The correlations of anteroposterior postural balance and muscle strength of both lower extremities in individuals with unilateral THR were analyzed. Result: (1) Significant increases in the standard deviation of the sway path in sagittal plane and the area of path were observed in patients either open or close eyes (P〈 0.05), and a significant increase in the standard deviation of the sway path in coronal plane was observed in patients with eyes opened (P 〈 0.05), when compared to controls. (2)Isokinetic muscle strength of hip and knee flexor-extensor in affected side significantly decreased compared to those of non-affected side (P 〈 0.05). (3)The standard deviation of sway path in A/P plane was inversely related to the ratio between operated and non-operated hip flexor strength, and the ratio between operated and non-operated knee extensor strength, and strength of hip and knee flexor and extensor in affected side (r=-0.446-0.652, P 〈 0.05). (4)The hip flexor muscle strength was a main strength factor to predict postural balance in A/P plane. Conclusion: Postural balance in the sagittal plane is correlated with the strength of hip flexor closely.
