腹腔镜肾上腺部分切除术治疗良性肾上腺肿瘤疗效研究

摘　　要：目的探讨腹腔镜肾上腺部分切除术治疗良性肾上腺肿瘤的疗效。方法选择良性肾上腺肿瘤患者60例作为研究对象，所有患者在手术前已通过生化检查、临床表现以及影像学资料确诊，同时于术后经过病理证实，采取随机的方式将其分为对照组和观察组，每组30例，其中对照组采取腹腔镜肾上腺全切除术，观察组采取腹腔镜肾上腺部分切除术，对比分析两组患者的住院时间、手术时间、失血量以及并发症等。结果观察组手术时间和住院时间明显短于对照组，差异有统计学意义（P〈0．05）。在失血量方面，二者比较差异未见统计学意义（P〉0．05）。观察组有2例患者发生并发症，其并发症发生率为6．7％，对照组有7例患者发生并发症，其并发症发生率为23．3％，二者比较差异有统计学意义（P〈0．05）。结论在良性肾上腺肿瘤的临床治疗中，采用腹腔镜肾上腺部分切除术进行治疗，所获疗效显著且良好，可有效防治肾上腺功能代偿不全。Objective To investigate the effect of laparoscopic adrenal resection on benign adrenal tumors. Methods Sixty patients with benign adrenal tumor were chose as the research objects, all patients preoperatively has passed the biochemical examination, clearly diagnosed by clinical manifestation and imaging data, after postoperative pathology confirmed at the same time, all the patients were randomly divided into control group and observation group, each group with 30 patients. The control group were treated with laparoscopic total adrenal gland resection, the observation group were treated with laparoseopic part adrenal gland resection. The average length of hospital stay, average operation time, blood loss and complications of the two groups were compared. Results The average operation time and hospitalization time in observation group was obviously shorter than those in the control group, there were significant differences（P 〈0. 05）. There was no significant difference in the average blood loss （P 〉 0. 05 ）. In observation group, two patients had complications, the incidence of complications was 6. 7%. In the control group, 7 patients had complications, the incidence of complications was 23.3%, there was significant difference （ P 〈 0. 05 ）. Conclusions In the clinical treatment of benign adrenal tumors, laparoscopic part adrenal resection treatment has good curative effect, which can effectively prevent gland compensatory dysfunction.
