蔗糖铁联合促红细胞生成素治疗维持性血液透析肾性贫血

摘　　要：目的:观察蔗糖铁注射液联合促红细胞生成素治疗维持性血液透析肾性贫血的临床疗效。方法:184例维持性血液透析肾性贫血患者随机分为观察组和对照组。两组患者继续透析治疗,同时予补钙和降血压治疗。在此基础上,观察组予蔗糖铁注射液联合促红细胞生成素,对照组予硫酸亚铁缓释片联合促红细胞生成素治疗。根据患者血红蛋白含量调整药品剂量,均治疗8周。比较两组患者治疗前后血液相关指标和肾功能指标的变化,观察治疗期间药品不良反应发生情况。结果:治疗后4周和8周,两组患者血红蛋白含量、红细胞比容和血清铁水平等血液指标,以及血清肌酐、血尿素氮水平等肾功能指标,均较治疗前有明显好转(P<0.05),且观察组各项指标均明显优于对照组同期(P<0.05)。两组均未见药品不良反应发生。结论:静脉注射蔗糖铁联合促红细胞生成素治疗维持性血液透析肾性贫血能有效改善患者贫血症状和肾功能,且安全性好。Objective： To observe the curative effect of erythropoietin combined with iron sucrose in the treatment of hemodialysis renal anemia. Methods： One hundred and eighty four cases hemodialysis patients with renal anemia were randomly divided into two groups. The patients of observation group were given intravenous erythropoietin combined with iron sucrose for treatment,patients of control group were given intravenous erythropoietin combined ferrous sulfate sustained-release tablets. According to the hemoglobin content of the patients to adjust the drug dose,the course of treatment was 8 weeks. The changes of related indexes of blood and renal function indexes before and after treatment on the two groups were compared,the adverse drug reactions during treatment were observed. Results： After 4 weeks,8 weeks of therapy,the hemoglobin,indicators hematocrit,serum iron levels,serum creatinine,blood urea nitrogen of the patients in two groups all had obvious improvement than before（ P〈0. 05）,and those indexes in patients of the observation group were much better than that of control group（ P〈0. 05）. There were no adverse drug reaction in the two groups during treatment.Conclusion： Intravenous iron sucrose combined with erythropoietin therapy in hemodialysis renal anemia can improve iron absorption efficiency and improve patient symptoms of anemia and kidney function.
