正常胎儿肺动脉多普勒参数与孕周增长关系的初探

摘　　要：目的:探讨妊娠中、晚期正常胎儿肺动脉多普勒参数随孕周的变化规律。方法:回顾性分析从182例正常单胎孕妇中获得的192例次胎儿左(或右)肺动脉多普勒测量结果。所有多普勒测量均由同一名检查者连续重复测量3次,多普勒参数加速时间(AT)、射血时间(ET)、AT/ET比值、收缩期峰值流速(PSV)、舒张末期流速(EDV)、平均血流速度(AV)、搏动指数(PI)和阻力指数(RI)取平均值记录。应用变异系数(CV)分析观察者内各多普勒参数的稳定性,应用Pearson相关分析探讨各参数与孕周的关系。结果:多普勒参数AT、ET、AT/ET值、PSV、EDV、AV、PI和RI的观察者内CV分别为(4.49±2.97)%、(2.56±1.32)%、(4.32±2.64)%、(4.50±3.31)%、(13.07±7.88)%、(4.74±2.76)%、(6.33±3.64)%和(2.67±1.50)%。妊娠中、晚期正常胎儿肺动脉多普勒参数AT和AT/ET值均与孕周呈正相关(P<0.001),但ET、PSV、EDV、AV、PI和RI均与孕周无相关性(P>0.05)。结论:多普勒测量胎儿左(右)肺动脉可用于评估胎儿肺循环,除AT和AT/ET值外,孕龄并不影响妊娠中、晚期正常胎儿肺动脉多普勒参数。Objective： To explore the correlation between the Doppler parameters in fetal pulmonary artery and gestational age in the normal fetuses during the second and third trimester. Methods： This retrospective study included 192 Doppler examinations of the left（ or right） pulmonary artery in 182 normal fetuses. All Doppler measurements were repeated 3 times by a single operator,and the Doppler parameters including acceleration time（ AT）,ejection time（ ET）,AT/ET ratio,peak systolic velocity（ PSV）,end-diastolic velocity（ EDV）,average velocity（ AV）,pulsatility index（ PI） and resistance index（ RI） were recorded. For each parameter,coefficient of variance（ CV） was used to evaluated the stability among observers,and pearson correlation analysis was used to explore the relationships with gestational age. Results： The CV of Doppler parameters including AT,ET,AT / ET ratio,PSV,EDV,AV,PI and RI was（ 4. 49 ± 2. 97） %,（ 2. 56 ± 1. 32） %,（ 4. 32 ± 2. 64） %,（ 4. 50 ± 3. 31） %,（ 13. 07 ±7. 88） %,（ 4. 74 ± 2. 76） %,（ 6. 33 ± 3. 64） % and（ 2. 67 ± 1. 50） %,respectively. With increase of gestational age a significant increase of AT and AT / ET ratio was observed（ P 〈0. 001）,but other parameters including ET,PSV,EDV,AV,PI and RI showed no changes（ P 〉0. 05）. Conclusions： Doppler examination of fetal pulmonary artery is feasible for the assessment of pulmonary circulation. All fetal pulmonary arterial Doppler parameters,except AT and AT / ET ratio,were not significantly altered during the second and third trimester in the normal fetuses.
