摘 要:目的:探讨小剂量多巴胺与羟乙基淀粉溶液联合治疗重度卵巢过度刺激综合征的临床疗效。方法:选择重度卵巢过度刺激综合征(OHSS)患者60例,使用随机数字法分为两组,对照组30例采用羟乙基淀粉溶液500-1000 ml/d,静脉滴注10-14天,同时采用白蛋白加胸腹腔积液抽吸治疗;治疗组30例在对照组治疗方法的基础上同时加用小剂量多巴胺40 mg/d(2μg/kg·min),静脉滴注,用药7-10天。观察并比较两组患者血生化相关指标及住院时间、住院费用等的差异。结果:治疗组总蛋白、白蛋白的疗效优于对照组,差异有统计学意义(P〈0.01)。治疗组尿量恢复时间、血细胞比容恢复时间、住院时间明显比对照组短,差异有统计学意义(P〈0.01,P〈0.05)。治疗组住院费用明显比对照组低,差异有统计学意义(P〈0.01)。两组治疗后均能有效改善OHSS症状、体征和血液生化指标。白细胞、尿素氮、肌酐、丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、钾、钠治疗前后比较,差异无统计学意义(P〉0.05)。结论:重度OHSS患者应用小剂量多巴胺与羟乙基淀粉溶液联合治疗,能迅速缓解症状,缩短住院时间,降低住院费用及减少并发症的发生,为治疗OHSS的一种有效方法。Objective: To explore the clinical effect of low dose dopamine and intravenoushydroxyethyl starch on severe ovarian hyperstimulation syndrome( OHSS). Methods: 60 cases with severe OHSS were enrolled and randomly divided into two groups. 30 patients in control group were treated with intravenous hydroxyethyl starch( HES) 500 -1000 ml/d for 10 to 14 days,and albumin combined with aspiration of pleural effusion and seroperitoneum. 30 patients in treatment group were treated with low dose dopamine 40 mg / d[2 μg /( kg·min) ] for 7 to10 days,as well as the treatment in control group. The changes in blood biochemical parameters,hospitalization time and cost were compared between two groups. Results: The levels of total protein and albumin in treatment group were significantly higher than those in control group( P〈0. 01). The urine volume recovery time,hematocrit recovery time,and hospitalization time in treatment group were significantly shorter than those in control group( P〈0. 01,P〈0. 05). The hospitalization cost in treatment group was significantly lower than that in control group( P〈0. 01). The symptoms,signs of OHSS and blood biochemical parameters improved in both two groups. There was no significant difference in the level of white blood cell,urea nitrogen,creatinine,potassium or sodium between both groups( P〈0. 05). Conclusions: Low dose dopamine and intravenous hydroxyethyl starch can quickly improve the symptoms,shorten the hospitalization time,reduce hospitalization costs and reduce complications of OHSS,suggesting it should be an effective approach for OHSS.
