摘 要:目的:探讨和分析高通量透析对维持性血液透析患者甲状旁腺激素及左心功能的影响。方法选择2012年11月~2014年11月在我院透析中心接受维持性血液透析患者186例,分为高通量血液透析组(HFHD 组)和常规血液透析组(HD 组),HFHD 组采用高通量血液滤过器,HD 组采取低通量血液滤过器,比较分析两组患者透析治疗前后血清溶质下降率和左心室结构与功能。结果两组患者透析前血清尿素氮、血清肌酐和甲状旁腺激素的差异无统计学意义(P 〉0.05);透析后两组患者血清尿素氮和甲状旁腺激素的差异有统计学意义(P 0.05)。两组患者透析治疗后,左心室重量(LVM)、左心室的直径(LVD)、TE 峰和二尖瓣前叶 E 峰与室间隔左室面的距离(EPSS)均减少,左心室射血分数(LVEF)和 A 峰显著提高,HFHD 组与 HD 组比较,差异有统计学意义(P 〈0.05或 P 〈0.001)。结论高通量透析能更好地去除甲状旁腺激素,很好地改善心脏功能,具有临床应用价值。Objective To study the effect of high flux dialysis on parathyroid hormone and left ventricular function in maintenance hemodialysis patients. Methods A total of 186 cases of maintenance hemodialysis patients who were cared at our hospital from November 2012 to November 2014 were selected as the research subjects.All of patients were randomly divided into high flux hemodialysis (HFHD)group treated with high flux dialysis and regular hemodialysis (HD)group treated with low flux dialysis.Then,a comparative analysis of pre-and post-treatment serum solute decreasing and left ventricular construction and function was done in the two groups. Results There was no significant difference between the two groups in blood urea nitrogen, serum creatinine and parathyroid hormone(P 〉0.05)before dialysis.After dialysis,comparison of the levels of blood urea nitrogen and parathyroid hormone yielded statistically significant differences lower in HFHD group than those in HD group (P 0.05)after dialysis.In addition,left ventricular mass (LVM),left ventricular diameter (LVD),TE-point,E-point of septal separation (EPSS)were decreased,and left ventricular ejec-tion fraction (LVEF),A-point were significantly increased in the two groups after dialysis.Moreover,group-paired comparison yielded statistically significant differences (P 〈0.05 or P 〈0.001 ). Conclusion The results suggested that high flux dialysis could improve the elimination of the parathyroid hormone and the left ventricular function,be worthy of further application in clinical practice.
