摘  要:目的:在新医改和城镇化的持续推进下,基于健康公平理论和卫生筹资公平性,研究实施城乡居民医保一体化对财政筹资负担的影响情况,为提高城乡卫生筹资与健康公平性提供参考意见。方法:收集相关文献,比较北京市城乡居民医疗筹资差别,分析相关数据,研究实施城乡居民医保一体化筹资对财政筹资负担如何。结果:人均筹资水平不变,人均缴费98.2元,低于城镇居民基本医疗保险和新农合人均缴费水平,个人和财政均能承受筹资负担(方案1)。适当提高筹资标准,合理规划筹资结构,不会严重增加财政负担(方案5)。结论:通过对比北京市城乡医保筹资情况,一体化筹资确有一定的必要性和有利性,做好制度设计,合理筹资可保证筹资的安全可持续性。Objective To study the influence of integration of urban basic medical insurance system (URBMI) and rural new cooperative medical system (NCMS) on the burden on financing, and provide references for improving the equity of health and health financing. Methods Related literatures were collected to comparatively study the differences of financing. Results The per ca- pita financing kept unchanged, which was bearable and lower than the average financing of URBMI and NCMS. Limited increase in financing standard will not significantly increase the financial burden. Conclusion It is necessary and beneficial to implement inte- gration of financing,which needs to be rationally designed to ensure its safety and sustainability.
