核素显像评价2型糖尿病胃肠运动障碍研究

摘　　要：目的应用核素显像检测2型糖尿病(T2DM)患者胃肠运动障碍,并探讨其相关代谢危险因素。方法选择2012年6月至2013年12月在第三军医大学大坪医院野战外科研究所高血压内分泌科住院的T2DM患者15例(糖尿病组),健康志愿者18名(对照组)。收集一般资料,测定生化指标、四肢神经诱发电位。通过核素显像测定胃排空及小肠运动时间。分析组间运动时间、一般资料、生化指标、影响胃排空及小肠运动时间的独立危险因素。结果与对照组相比,糖尿病组胃排空及小肠运动时间明显延长;口服葡萄糖耐量试验2 h血糖、总胆固醇(TC)及三酰甘油(TG)是T2DM患者胃排空及小肠运动时间延长的独立危险因素。结论应用核素显像可及早发现T2DM患者胃肠运动障碍;预防T2DM患者胃肠运动障碍,不仅要积极控制血糖,还需严格控制血脂。Objective To detect the gastrointestinal motility dysfunction using nuclear imaging in patients with type 2 diabetes mellitus（T2DM） and explore the related risk factors. Methods A total of 15 patients with T2DM and 18 healthy volunteers were involved in this study. General data, serum biochemical indices and limb nerve evoked potentials were measured. The gastric emptying time and intestinal peristalsis time were calculated using nuclear imaging. Student＇s t tests were applied to investigate the differences of gastrointestinal peristalsis time, general data and biochemical factors between groups. Multiple stepwise regression analysis was performed to assess the independent risk factors of gastrointestinal peristalsis time. Results The gastrointestinal peristalsis time of the T2DM patients were extremely longer than those of the healthy volunteers. The independent risk factors of gastrointestinal motility time were 2h plasma glucose during oral glucose tolerances test（OCTT 2h PG）, total cholesterol（TC） and triglyceride（TG）. Conclusion Gastrointestinal motility dysfimction could be detected timely by nuclear imaging. In order to prevent the gastrointestinal motility dysfunction in patients with T2DM, both plasma glucose and lipid should be strictly controlled.
