阿司匹林联合氯吡格雷治疗短暂性脑缺血发作的临床观察

摘　　要：目的:观察阿司匹林联合氯吡格雷治疗短暂性脑缺血发作(TIA)的临床疗效及安全性。方法:将82例TIA患者随机分均为观察组和对照组。两组患者均给予常规治疗。与此同时,对照组患者给予阿司匹林片100 mg,口服,qd;观察组患者在对照组治疗的基础上加用氯吡格雷片75 mg,口服,qd。两组患者疗程均为14 d。观察两组患者的临床疗效,治疗前后血液流变学指标、脑氧代谢指标[颈内静脉血氧饱和度(SjvO2)、脑静脉血氧含量(CjvO2)、动脉-颈内静脉血氧含量差(Da-jvO2)、动脉氧含量(CaO2)]及不良反应发生情况。结果:观察组患者总有效率显著高于对照组,差异有统计学意义(P<0.05)。治疗后,两组患者血液流变学指标、Da-jvO2、CaO2均显著低于同组治疗前,且观察组低于对照组;SjvO2、CjvO2均显著高于同组治疗前,且观察组高于对照组,差异均有统计学意义(P<0.05或P<0.01)。两组患者治疗期间均未见明显不良反应发生。结论:短期应用阿司匹林联合氯吡格雷治疗TIA较单用阿司匹林疗效更显著,安全性较好。OBJECTIVE：To observe the clinical efficacy and safety of aspirin combined with clopidogrel in the treatment of transient ischemic attack（TIA）.METHODS：Totally 82 patients with TIA were randomly divided into control group and observation group.The patients were given routine treatment.Meanwhile,the patients in control group were orally given aspirin 100 mg,once a day;and patients in observation group were orally added clopidogrel 75 mg based on the treatment of control group,once a day.The course of both was 14 d.The clinic data was observed,including clinical efficacy,hemorheological indexes,cerebral oxygen metabolism index（SjvO2,CjvO2,Da-jvO2 and CaO2）before and after treatment and ADR.RESULTS：The total efficacy rate in observation group was significantly higher than control group（P〈0.05）.After treatment,the hemorheological indexes,Da-jvO2 and CaO2 in 2 groups were all significant higher than before,and observation group was higher than control group;SjvO2 CjvO2were significantly higher than before,and observation group was higher than control group（P〈0.05 or P〈0.01）.There were no obvious adverse reactions during the treatment.CONCLUSIONS：Short-term use of aspirin combined with clopidogrel have obvious efficacy in the treatment of TIA with good safety.
