摘 要:目的观察煤矿井下工人职业所致膝关节慢性滑囊炎滑膜病变在超声中的表现。方法通过高频超声及彩色多普勒超声对矿工组(43例)及正常对照组(33例)的膝关节髌上囊及髌前囊滑膜厚度、滑膜彩色血流分级情况进行对比,总结煤矿井下工人职业所致膝关节慢性滑囊炎滑膜病变的超声表现。结果所有矿工膝关节滑膜均有超声阳性表现:(1)髌上囊滑膜病例组较对照组增厚,血流信号增多(病例组血流分级为1~2级的13例,3级的3例;对照组均为0级);(2)髌前囊滑膜病例组较对照组增厚(病例组厚度均〉0.5 mm,对照组可测量的最大厚度≤0.3 mm),血流信号增多(病例组血流分级为1~2级的10例,3级的2例;对照组均为0级);(3)病例组髌上囊及髌前囊滑膜厚度优势侧较非优势侧增厚(P〈0.05),血流分级双侧差异无统计学意义(P〉0.05)。结论矿工慢性膝关节滑囊炎滑膜较对照组有明显改变,超声能较准确地发现和评价职业病所致膝关节滑膜病变,可为临床诊断慢性滑囊炎提供客观的依据。Objective The aim of this study was to explore the application value of ultrasound on synovial lesions in occupational knee joint chronic bursitis among miners. Methods The miner group(43 cases) and the control group(33 cases) were examined by high frequency and color Doppler ultrasound in synovial thickness, color flow grading of synovium of suprapatellar bursa and prepatellar bursa. The sonographic findings were analyzed. Results Positive synovial sonographic findings were seen in the knee joint among all miners. The synovium of suprapatellar bursa in the miner group was thicker than that in the control group.The color flow of synovium increased(13 were classified as grade one or two, and three were classified as grade three in miner group;all were classified as grade zero in the control group). The synovium of prepatellar bursa in the miner group was thicker than that in the control group(it was equal or lesser than 0.3 mm in the control group; it was greater than 0.5 mm in the miner group). The color flow of synovium increased(10 were classified as grade one or two, and two were classified as grade three in the miner group; all were classified as grade zero in the control group). The synovium of suprapatellar bursa and prepatellar bursa in dominant side were thicker than that in non-dominant one(P〈0.05), with no statistical significance between the two in color flow grading of synovium(P〉0.05). Conclusion The synovium of knee joint in the miner group was obviously different than that in the control group. High frequency color ultrasound is valuable in detecting synovial lesions and adjuvant diagnosis in occupational knee joint chronic bursitis.
