摘 要:目的探讨ELISA法检测丙型肝炎病毒(HCV)抗体诊断阳性的最佳临界值(Cut-off值)及其可疑区间,为不同实验室间的结果互认提供依据。方法选取某院住院或体检的130例经ELISA法初筛为阳性的血清标本和25例接近临界值(阴性)标本(OD=0.15~2.0),采用重组免疫印记(RIBA)法检测确认其阴、阳性。利用SPSS17.0绘制ROC曲线,确定最佳Cut-off值,进一步探讨可疑区间。结果 ELISA法检测HCV抗体诊断阳性的最佳Cut-off值为0.2513,其可疑区间范围为0.2136~0.5513。结论确定适合本实验室最佳Cut-off值和可疑区间能有效地降低丙型肝炎抗体检测的漏诊率和误诊率。Objective The study assessed the optimal cut-off value and its dubious interval of ELISA in detection of anti-HCV antibody, aiming to provide a reference for the mutual reliability of test results from different laboratories. Methods 130 serum specimens tested positive of anti-HCV antibody in a preliminary ELISA screening from hospitalized patients or physical examinees at our hospital were selected; meanwhile, 25 specimens tested negative or close to the cut-off value(OD=0.15-2.0) were also selected. Recombinant Western Blot(RIBA) was used to confirm their positivity and negativity. An ROC curve was prepared by using SPSS17.0 to identify the optimal cut-off value and explore the dubious interval. Results The optimal cut-off value of ELISA in detecting anti-HCV antibody was 0.2513, with a dubious interval ranging from 0.2136 to 0.5513. Conclusion Confirming the optimal cut-off value and dubious interval suitable for our laboratory can effectively help reduce the incidences of missed diagnosis and misdiagnosis in detection of anti-HCV antibody.
