人乳头状瘤病毒L1壳蛋白预测宫颈上皮内瘤变病变程度及转归的价值研究

摘　　要：目的探讨人乳头状瘤病毒L1蛋白(HPV L1壳蛋白)预测宫颈上皮内瘤变程度及转归的价值以指导宫颈病变的临床处理。方法选择在漯河医学高等专科学校第一附属医院就诊的高危型HPV(HR-HPV)阳性的312例妇女,进行TCT检查并行HPV L1蛋白检测,对病理证实为宫颈上皮内瘤变(CIN)的192例妇女随访2年。结果最终病理证实CINⅠ92例,CINⅡ63例,CINⅢ37例。CINⅠ患者中HPV L1壳蛋白阳性率高于CINⅡ、CINⅢ(P<0.05),HPV L1壳蛋白阳性率与宫颈疾病的严重程度呈负相关(rs=-0.325)。未手术组中,HPV L1蛋白(+)患者的病变转阴率高于LHPV L1蛋白(-)患者,差异有统计学意义(P<0.05)。CINⅠ中,HPV L1蛋白(+)患者的病变转阴率高于LHPV L1蛋白(-)患者(P<0.05),CINⅡ中,未手术组HPV L1蛋白(+)患者的病变转阴率高于LHPV L1蛋白(-)患者(P<0.05)。结论结合HR-HPV检查和宫颈细胞学检查,HPV L1壳蛋白检测可以预测宫颈上皮内瘤变程度及其临床转归。Objective The aim of this study was to explore the value of HPV L1 capsid protein test in forecasting the degree of pathological changes of cervical intraepithelial neoplasia and its outcome, in order to direct the corresponding clinical procedures.Methods Three hundred and twelve cases of women with HR-HPV infection were chosen. And the patients all received the TCT test regarding, HPVL1 capsid protein. And the follow-up study was done to 192 women who were diagnosed with CIN by pathological examination. Results Final pathological examination confirmed that there were 115 CIN I cases, 54 CIN II cases, and29 CIN III cases. The positive rate for HPV L1 capsid protein expression in CIN I group was higher than CIN II group and CIN IIIgroup（P〈0.05）. The HPV L1 capsid protein expression decreased with increasing histopathological diagnosis（rs=-0.316,P〈0.05）. The disappearing rate for cervical intraepithelial neoplasia of HPV L1（＋） was higher than HPV L1（-） in no-operation group and CIN I（P〈0.05）, and the disappearing rate for cervical intraepithelial neoplasia of HPV L1（＋） was higher than HPV L1（-）in CIN II without operation（P〈0.05）. Conclusion On the basis of the testing of HR-HPV DAN and TCT, HPV L1 capsid protein had an important predictive value for cervical lesions and outcome.
