摘  要:目的检测胆固醇酯转运蛋白(CETP)基因D442G多态性与颈动脉粥样硬化斑块稳定性之间的相关性及临床意义。方法 2013.2-2014.1年某院功能科首次发现有颈动脉斑块者5 075例为病例组,以同期无动脉斑块者5 000例为对照组。应用聚合酶链反应(PCR)分析技术结合DNA直接测序技术,对研究对象CETP基因D442G(A/G)多态位点进行分型,按等位基因A和G的出现情况分为AA、AG、GG 3种基因型。采用非条件Logistic回归分析统计CETP基因多态位点与颈动脉粥样硬化斑块稳定性的相关性,同时检测研究对象的血脂水平及BMI等指标。结果病例组血脂指标中TC、TG、LDL-C、家族史与对照组比较明显升高,而HDL-C则明显低于对照组(均P〈0.05)。AA、AG、GG基因型在病例组中的分布频率分别为92.20%,6.50%,1.30%,在对照组中分别为98.28%,1.62%,0.10%,2组间基因型频率分布有明显差异(χ^2=9.572,P〈0.05)。多因素回归分析研究发现,CETP基因D442G(A/G)多态位点G变异等位基因与颈动脉粥样硬化斑块稳定性有显著的相关性(OR=5.97,95%CI=3.29~10.18,P〈0.05)。结论胆固醇酯转运蛋白(CETP)D442G(A/G)位点基因突变可能是影响颈动脉粥样硬化斑块不稳定性的一个重要原因。Objective The objective of the study was to investigate the correlation between and clinical significance of D442 G polymorphism of the cholesteryl ester transfer protein(CETP) gene and the stability of carotid atherosclerotic plaques. Methods5075 patients who were confirmed having carotid atherosclerotic plaques for the first time by the functional medicine department of our hospital between February of 2013 and January of 2013 were selected as the case group, and 5000 patients without carotid atherosclerotic plaques at our hospital during the same time period were selected as the control group. D442G(A/G) polymorphic loci of the CETP gene were genotyped by adopting PCR and direct DNA sequencing in conjunction, and were classified into AA,AG, and GG genotypes based on the occurrence of allele A and G. Unconditional logistic regression analysis was used to statistically analyze the correlation between the polymorphic loci of the CETP gene and the stability of carotid atherosclerotic plaques; blood lipid level and body mass indexes(BMIs) of the subjects were also measured. Results The levels of TC, TG, and LDL-C and family history of the case group were significantly higher than those of the control group(P〈0.05), while HDL-C in case group was lower than that in control group(P〈0.05). The distribution frequencies of AA, AG, and GG genotypes in the case group were 92.2%,6.50%, and 1.30%, respectively; and were 98.28%, 1.62%, and 0.10%, respectively, in the control group; the difference in distribution frequencies between the two groups was statistically significant(χ^2=9.572, P〈0.05). Multivariate regression analysis showed that G mutant allele at the D442G(A/G) polymorphic locus of the CETP gene was significantly correlated with the stability of carotid atherosclerotic plaques(OR=5.97, 95%CI=3.29-10.18, P〈0.01). Conclusion Gene mutation at the D442G(A/G) locus of the CETP gene may be an important factor affecting the instability of carotid atherosclerotic plaques.
