摘 要:目的探讨初诊急性髓细胞白血病(AML)感染患者外周血CD4+CD25+FOXP3+Treg细胞的表达,以降低AML感染率。方法选取2010年2月-2014年6月初诊AML感染患者36例,设为AML感染组,36例患者中12例化疗后完全缓解,设为缓解组,将余下24例患者设为复发组,AML感染组36例患者依据FAB分型标准,M2 6例、M3 17例、M4 7例、M5 6例;选择20例单纯初诊AML患者为AML组,另选择健康人群20名为对照组;检测受试者CD4+CD25+调节性T细胞表达情况;使用SPSS19.0软件进行统计分析。结果 AML感染组CD4+CD25+FOXP3+Treg及CD4+FOXP3+T细胞比例高于AML组和对照组,差异有统计学意义(P<0.01),而AML组CD4+CD25+FOXP3+Treg及CD4+FOXP3+T细胞比例高于对照组,差异有统计学意义(P<0.01);复发组CD4+CD25+Treg及CD4+FOXP3+T细胞比例高于缓解组,差异有统计学意义(P<0.01);初诊AML感染患者外周血CD4+CD25+FOXP3+Treg细胞比例和CD4+FOXP3+T细胞比例呈现正相关(r=0.387,P=0.015)。结论初诊AML感染患者CD4+CD25+细胞比例高于健康人群和单纯AML患者。OBJECTIVE To explore the expression of CD4 + CD25 + FOXP3+ Treg cells in peripheral blood of the newly diagnosed acute myeloid leukemia (AML) patients with complicated infections so as to reduce the incidence of infections in the AML patients .METHODS A total of 36 newly diagnosed AML patients with complicated infec‐tions who were treated in the hospital from Feb 2010 to Jun 2014 were assigned as the AML infection group ,and 12 of the 36 patients got complete remission and were set as the remission group ,the rest of 24 cases were as‐signed as the recurrence group;the 36 patients in the AML infection group were divided into the M2 with 6 cases , M3 with 17 cases ,M4 with 7 cases ,and M5 with 6 cases ;totally 20 patients with single newly diagnosed AML were chosen as the AML group ,and 20 healthy people were set as the control group .The expression of the CD4 +CD25 + regulatory T cells of the participants were detected ,and the statistical analysis was performed with the use of SPSS19 .0 software .RESULTS The proportions of the CD4 +CD25 + FOXP3+ Treg and CD4 + FOXP3+ T cells of the AML infection group were significantly higher than those of the AML group and the control group (P〈0 .01) ,while the proportions of the CD4 +CD25 + FOXP3+ Treg and CD4 + FOXP3+ T cells of the AML group were significantly higher than those of the control group (P〈 0 .01) .The proportions of the CD4 + CD25 + Treg and CD4 + FOXP3+ T cells of the recurrence group were significantly higher than those of the remission group(P〈0 .01) .The proportion of the peripheral blood CD4 + CD25 + FOXP3+ Treg cells of the newly diagnosed AML pa‐tients with complicated infections was positively correlated with the proportion of the CD4 + FOXP3+ T cells (r=0 .387 ,P=0 .015) .CONCLUSION The proportion of the CD4 + CD25 + cells of the newly diagnosed AML patients with complicated infections is higher than that of the healthy population and the patients with single AML .
