不同剂量盐酸氨溴索对肺癌术后患者肺功能和并发症的影响  

Impact of Different Doses of Ambroxol Hydrochloride on Pulmonary Function and Complications in Postoperative Patients with Lung Cancer Surgery

摘　　要：目的探讨肺癌术后氨溴索的最佳剂量方案。方法选取在医院进行肺癌手术的患者164例,将其分成4组,各41例。4组患者应用的盐酸氨溴索剂量为大剂量组(A组)600 mg/d,中剂量组(B组)300 mg/d,常规剂量组(C组)90 mg/d,低剂量组(D组)60 mg/d,连续用药5 d。分别在第1,3,5天观测各组C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、中性粒细胞绝对值(ANC)、动脉血氧分压(Pa O2)、氧合指数(Pa O2/Fi O2),观察5 d内肺癌术后的并发症及不良反应。结果治疗后,A组和B组的肺功能指标(CRP,TNT-α,ANC,Pa O2,Pa O2/Fi O2)在第5天均显著优于C组和D组(P<0.05);A组并发症肺部感染和肺不张的发生率分别为4.88%和2.44%,B组为7.32%和4.88%,C组为19.51%和17.07%,D组为24.39%和21.95%,A组和B组显著低于C组和D组(P<0.05);4组不良反应发生率比较,差异无统计学意义(P>0.05)。结论采用大剂量或中剂量盐酸氨溴索治疗肺癌术后患者,可改善肺功能和防治术后并发症,值得临床推广。Objective To study the therapeutic scheme of optimal dose of ambroxol after lung cancer surgery. Methods Totally 164 pa-tients with lung cancer surgery admitted to our hospital were selected and randomly divided into 4 groups,41 cases in each group. The doses of ambroxol hydrochloride in the four groups were the large dose of 600 mg/d （ group A ） ,middle dose of 300 mg/d （ group B ） , routine dose of 90 mg/d （ group C ） and the low dose of 60 mg/d （ group D ） for continuous 5 d. CRP,TNF-α,absolute neutrophil count（ANC）,PaO2,PaO2/FiO2 on 1,3,5 d in each group were observed. The postoperative complications of lung cancer and adverse re-actions within 5 d were observed. Results The pulmonary function indexers（CRP,TNF-α,ANC,PaO2,PaO2/FiO2）on 5 d in the group A and group B were significantly superior to those in the group C and group D（ P ﹤ 0. 05）;the occurrence rates of complicating pul-monary infection and atelectasis were 4. 88% and 2. 44% in the group A,7. 32% and 4. 88% in the group B,19. 51% and 17. 07%in the group C,24. 39% and 21. 95% in the group D respectively,which in the group A and group B were significantly lower than those in the group C and group D（ P ﹤ 0. 05）;the occurrence rate of adverse reactions had no statistically significant difference among 4 groups. Conclusion Adopting large dose or middle dose of ambroxol hydrochloride for treating postoperative patients with lung can-cer,improving their lung function and preventing the postoperative complications,and is worthy of clinical promotion.
