摘 要:目的探讨神经行为发育监测及早期干预对缺血缺氧性脑病新生儿智能发育的影响。方法将150例缺血缺氧性脑病新生儿分为早期干预组和对照组。早期干预组在综合治疗的基础上给予针对性的个性化早期干预。比较两组患儿早期神经行为评分法(NBNA)、治疗3个月后智能发育指数(MDI)和运动发育指数(PDI)评分。结果治疗后两组患儿NBNA评分均显著提高,差异有统计学意义(P<0.05),但早期干预组患儿治疗后7d和14d NBNA评分均显著高于对照组患儿,差异有统计学意义(P<0.05);治疗3个月后不同时期,早期干预组患儿MDI和PDI评分均显著高于对照组患儿,差异有统计学意义(P<0.05);随访18个月后,早期干预组预后良好率(86.7%)显著高于对照组(72.0%),差异有统计学意义(P<0.05)。结论神经行为发育监测及早期干预对缺血缺氧性脑病新生儿具有重要的积极意义,可改善患儿预后,促进患儿智能发育。Objective To investigate the impact of neurobehavioral developmental monitoring and early inter‐vention on intellectual development in neonates with hypoxic‐ischemic encephalopathy .Methods 150 neonates with hypoxic‐ischemic encephalopathy were randomly divided into the early intervention group and the control group .The early intervention group was given the targeted personalized early intervention on the basis of comprehensive treat‐ment .The early Neonatal Behavioral Neurological Assessment (NBNA ) scores ,mental development index (MDI) scores after 3‐month treatment and psychomotive development index (PDI) scores were compared between the two groups .Results After treatment ,the NBNA scores in the two groups were significantly increased with statistical difference(P〈0 .05) ,but the NBNA scores on 7 ,14 d after treatment in the early intervention group were signifi‐cantly higher than the those in the control group ,the differenec was statistically significant (P〈0 .05) ,and the MDI and PDI socres in the different periods after 3‐month treatment in the early intervention group were significantly higher than those in the control group with statistical difference (P〈0 .05);After 18‐month‐following‐up ,the good prognosis rate in the early intervention group was 86 .7% ,which was significantly higher than 72 .0% in the control group ,the difference was statistically significant(P〈0 .05) .Conclusion Neurobehavioral developmental monitoring and early intervention has an important active significance for neonates with hypoxic‐ischemic encephalopathy ,can improve the prognosis and promote intellectual development .
