摘 要:目的分析乳腺局部结构紊乱的超声表现,探讨高频超声对乳腺局部结构紊乱良恶性病变的诊断价值。方法对82例术前超声诊断为乳腺局部结构紊乱患者的病理资料进行回顾性分析。结果病理结果为40例乳腺腺病,16例纤维腺瘤,15例浸润性癌,4例原位癌,2例肉芽肿性炎,2例导管内乳头状瘤,3例化脓性炎。边缘成角、微钙化、后方衰减及内部回声情况与局部结构紊乱的病理分类有相关性,差异有统计学意义(P<0.05)。ROC曲线分析结果 ,边缘有无成角(灵敏度60.9%,特异度91.5%,曲线下面积0.797)和有无微钙化(灵敏度68.2%,特异度93.3%,曲线下面积0.839)用于诊断局部结构紊乱良恶性有较高效能,且有无微钙化诊断效能最高。结论乳腺局部结构紊乱大部分为腺病等良性病变,但亦有恶变可能;边缘成角和微钙化是诊断乳腺局部结构紊乱良恶性有意义的特征,且有无微钙化诊断效能最高。Objective To analyze the ultrasonography of breast structure disorder in part and to discuss the value of of high -frequency ultrasound in the diagnosis of breast structure disorder in part. Methods Totally 82 cases of breast structure disorder in part were diagnosed by high - frequency ultrasound, and the results had retrospective study compared with post -operative pathological results. Results Marginal angulation, microcalcification, posterior attenuation and internal echoes had relevance to pathological results. Marginal angulation and microcalcification had more efficiency in diagnosis of benign and malignant of breast structure disorder in part, and microcalcification had the highest efficacy by ROC curve. Conclusion Marginal angnlation and microealcification had significant features in diagnosis of breast structure disorder in part, furthermore microcalcification had highest diagnosis efficacy.
