经皮置钉并经椎弓根伤椎植骨治疗胸腰椎骨折  

Percutaneous screw fixation through pedicle of vertebral arch and vertebral bone grafting in treatment of thoracic lumbar fracture

摘　　要：目的探讨经皮置钉并经伤椎椎弓根植骨治疗胸腰椎骨折的早期临床效果。方法回顾性分析郑州市骨科医院急诊科2010年6月份到2011年5月份应用经皮置钉结合经伤椎椎弓根植骨治疗胸腰椎骨折并成功随访的15例,分别测量伤椎术前,术后2周和1年前缘高度压缩比、后缘高度压缩比、Cobb角以及椎管占有率进行统计学分析。结果术后2周及1年拍片对比椎体前缘及后缘高度较术前均有明显增加,Cobb角较术前明显好转,CT示椎管占有率明显减少,差异在统计学上有显著性。术后1年椎体前缘及后缘的高度较术后2周轻微减低,Cobb角及椎管占有率略有增加。差异在统计学上没有显著性。结论经皮置钉结合经皮伤椎椎体内植骨治疗胸腰椎骨折具有经皮置钉术和经伤椎椎弓根椎体内植骨的优点,并没有增加手术操作的困难和并发症的发生率。早期观察效果较好。Objective To investigate the early clinical results of percutaneous transpedicular artificial bone in the treatment of thoracic and lumbar fracture. Methods A retrospective analysis of 2010 June to 2011 May in our department by percutaneous placement combined with vertebral pedicle bone graft in the treatment of 15 cases of fracture of thoracolumbar vertebra, they were measured both 2 weeks and 1 years after operations at the height of the front of the compression ratio, high compression ratio, Cobb trailing edge angle and the share of spinal canal. Results After 2 weeks and 1 years filming contrast vertebra body leading edge and trailing edge height with a preoperative were increased, Cobb was significantly improved, the difference is statistically significant, CT showed that the spinal share had significantly reduced. After 1 year, leading edge and trailing edge of the vertebral body height is slightly decreases , according to 2 weeks postoperatively. The Cobb angle and the spinal canal share were increased sightly. Differences were not statistically significant. Conclusion Percutaneous screw fixation combined with percutaneous vertebral body bone graft in treatment of thoracolumbar fractures with percutaneous screw placement technique and the vertebral pedicle vertebral body bone graft advantages, it did not increase the difficulty in operation and the incidence of complications. The early observation of effect is good.
