摘　　要：目的探讨肾细胞癌伴肉瘤样分化的临床及组织病理学特征。方法对4例经手术切除、病理证实的肾细胞癌伴肉瘤样分化的病例进行临床病理分析,并复习文献,讨论其临床、病理特征、治疗和预后。结果 3例由肾透明细胞癌和肉瘤样成分构成;1例由分化差的肉瘤样梭形细胞构成。4例中肉瘤样成分均表达角蛋白和波形蛋白。结论肾细胞癌伴肉瘤样分化时恶性程度高,预后不良,确诊依赖病理学及免疫组织化学检查。需要与原发肾肉瘤、癌肉瘤进行鉴别诊断。Objective To study the clinical and pathologic characteristics of Sarcomatoid renal cell carcinoma. Methods The clinical and pathologic findings were evaluated in 4 cases of biopsy confirmed Sarcomatoid RCC. The clincopathologic features,treatment and prognosis were discussed and literatures reviews. Results There were 3 cases composed of renal cell carcimoma and sarcomatoid, and 1 case was composed of spindle cell. 4 cases of sarcomatoid componemt were keratin and vimentin positive. Conclusions Sarcomatoid RCC is a highly malignant disease and associated with poor prognosis. The diagnosis relies on detailed histologic examination and immunohistochemical. More attention should be paid in the differential diagnosis of this tumor from nephrosarcoma and
