四肢关节专用MRI在隐性创伤性骨折中的价值  被引量:5

摘  要:目的探讨四肢关节专MRI诊断隐性创伤性骨折的价值。方法对54例四肢关节外伤后x线平片显示阴性患者行MRI检查,检查序列包括T1WI、T2WI、STIR,观察各种异常MRl征象。结果54例(56处)隐性骨折,其中T1wI对骨折线显示的阳性率为100%,T2WIN阳性率为82.1%,STIR的阳性率为92.6%,T1WI与T2WI、STIR序列比较差异有统计学意义。STIR对骨髓水肿显示较T1WI和T2WI清晰。MRI能同时显示关节周围软骨,韧带、肌腱、半月板的损伤情况。结论MRI是诊断四肢关节隐性创伤性骨折的最佳影像学检查方法,能清晰显示骨折线和髓腔水肿,对指导临床治疗和避免医疗纠纷有重要意义。Objective To discuss the value of the dedicated extremity MR/in diagnosing of occult traumatic fracture. Methods Fifty-four patients who experienced bone and joint injury with negative findings proved by X- ray films were performed MRI examinations, sequences include: T1- weighted imaging, T2- weighted imaging, and short time inversion recovery ( STIR ) imaging. The abnormal MRI appearances were retrospec- tively analyzed. Results 56 places occult fractures ( in 54 cases ) were found, sensitivity were 100 percent on TIWI, 82.1 percent on T2WI, and 92.6 percent on STIR. There was statistical significance in T1WI and TEWI, STIR sequence.Bone marrow edema showed more clearly on STIR than T1WI and T2WI.MRI can also display injury of the arthrodial cartilage, tendon, ligament, meniscus. Conclusion MRI is the best imaging method in diagnosis of bone and joint occult traumatic fracture, it can clearly show fracture line and bone marrow edema.MRI is of great value to guide the clinical treatment and to avoid medical disputes.
