摘 要:目的观察法舒地尔治疗缺氧性肺动脉高压的疗效。方法将58例缺氧性肺动脉高压的患者随机分为治疗组和对照组,对照组给予一般对症治疗;治疗组在对照组的基础上,加用法舒地尔30 mg静滴,1次/d,连续15 d。治疗前和疗程结束后,分别检测两组肺动脉收缩压、Pa O2、Sa O2及SBP,测定患者外周血内皮素-1(ET-1)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、N端脑钠素前体水平(NT-pro BNP)水平。结果治疗组治疗后肺动脉收缩压显著降低,且低于对照组治疗后肺动脉收缩压(P<0.05);两组治疗前后Pa O2、Sa O2、收缩压和舒张压无明显变化;治疗组治疗后血清ET-1、hs-CRP、TNF-α、NT-pro BNP水平较治疗前明显降低(P<0.05),而对照组治疗前后无显著性差异(P>0.05)。结论法舒地尔治疗缺氧性肺动脉高压,可以降低肺动脉收缩压,同时降低血清ET-1、hs-CRP、TNF-α水平。Objective To observe the curative effect of fasudil on hypoxic pulmonary hypertension (HPH). Methods 58 patients with HPH were randomly divided into treatment group and control group. The patients in the control group were provided with general symptomatic treatment; on the basis of the control group, intravenous infusion of fasudil of 30 mg was administered to the patients in the treatment group once per day, for 15 consecutive days. Before and after the treatment, the pulmonary artery systolic blood pressur(SBP), PaO2, SaO2 in the two groups were detected, and the peripheral blood endothelin-1 (ET-1), high sensitive C reactive protein (hs-CRP), tumor necrosis factor ct (TNF-α), and N-terminal pro-brain natriuretic peptide (NT-proBNP) levels were determined. Results After treatment, the pulmonary artery SBP of the patients in the treatment group decreased significantly, and were lower than that in the control group after the treatment (P 〈 0.05); before and after the treatment, PaO2, SaO2, and SBP and diastolic blood pressures (DBP) between the two groups had no obvious change; after treatment, the levels of ET-1, hs-CRP, TNF-α, and NT-proBNP in the treatment group were significantly lower than those before the treatment (P 〈 0.05), while those in the control group before and after treatment had no significant difference (P 〉 0.05). Conclusion In treatment of HPH, fasudil can reduce pulmonary artery SBP and the levels of serum ET-1, hs-CRP, and TNF-α.
