摘 要:临床实验室在医疗决策中扮演着重要角色。随着新技术的广泛应用,临床实验室检测的准确度和灵敏度得到极大提高,样本周转时间越来越短,医生对疾病的诊断和治疗越来越依赖临床实验室的检测结果。由于检验项目和检验方法的多样化和复杂化,对检验标本采集和保存的要求也越来越高,解读化验报告更加复杂。临床实验室不仅要做好检测,还有责任和义务保证得到的检测样本完全符合要求,并把测定结果准确传达到医生手中。临床实验室的质量管理分为三个过程,即分析前、分析中和分析后。现代临床实验室错误发生的常见环节已不在分析中,而是分析前的样本收集和处理过程,以及分析后的结果发送和解读过程。本文分析这三个过程中重要但又容易被忽略的一些质量管理细节。Clinical laboratories play very important roles in medical decisions. With the use of new technologies, the precision and accuracy of tests in clinical laboratories have been improved greatly. At the same time, the turn around time in clinical laboratories decreased from days to hours. On the other hand, clinical laboratories are facing more challenges in specimen processing and report interpretation due to the complicated methodologies and increased analytes. In addition to the analysis of patient specimens, the clinical laboratories are also responsible for processing patient samples correctly and interpreting testing results accurately. The total process in a clinical laboratory includes pre-analytical, analytical, and post- analytical phases. Accumulated evidence indicated that most clinical laboratory errors are found in pre- analytical and post-analytical phases. This review will discuss some details which are related to the quality of clinical laboratories and are easily ignored.
