无创呼吸机治疗重症哮喘12例临床效果分析

摘　　要：目的观察并分析无创呼吸机治疗重症哮喘的临床疗效,为临床上紧急治疗重症哮喘做出指导。方法抽取重症哮喘患者24例,将其随机分为对照组和观察组(n=12)。对照组患者采取常规的治疗方法,观察组患者在给予常规治疗的基础上加用无创双水平正压呼吸机(bi-level positive airway pressure,Bi PAP)鼻面罩正压通气,观察并分析2组患者的疗效。结果 2组患者在治疗后4 h后除p H值无明显改变外,心率、Pa CO2、Pa O2、呼吸频率都比治疗前有显著改善(P<0.05),观察组患者的改善程度更为显著,差异有统计学意义(P<0.05)。观察组患者应用糖皮质激素、茶碱类药物和吸入β2受体激动剂的比例显著低于对照组,差异有统计学意义(P<0.05)。结论无创双水平正压通气联合常规治疗可以显著缓解哮喘患者的症状,对急诊治疗重症哮喘发挥着重要的作用,值得临床推广和使用。Objective To observe and analyze of non-invasive ventilator clinical efficacy of severe asthma in the emergency treatment of diseases,to make urgent guidance for the clinical treatment of severe asthma.Methods 24 cases with severe asthma from February 2008 to February 2013 were selected and divided into the control group and the observation group randomly, 12 cases in each group. The control group was treated with conventional method, the observation group was given noninvasive positive pressure breathing machine double level （Blevel Positive Airway Pressure, BiPAP） nasal mask positive pressure ventilation on the basis of routine therapy. The curative effect of the two groups were observed. Results Two groups of patients after treatment of 4 hours in addition to the pH had no signiifcant change,heart rate and PaCO2, PaO2, PH, respiratory frequency is improved signiifcantly compared to that before treatment（P〈0.05）, observation group of patients more signiifcantly（P〈0.01）, improve the difference was statistically signiifcant （P〈0.05）. Observation group of patients used glucocorticoid, theophylline drugs and inhalation ofβ2 agonists was much lower than the control group, the difference between two groups was statistically signiifcant （P〈0.05）.Conclusion Noninvasive positive pressure ventilation in combination with conventional treatment can signiifcantly alleviate the symptoms of patients with asthma. It play an important role for emergency treatment of severe asthma which is worthy of clinical promotion and use.
