腹腔镜下完全腹膜外腹股沟疝修补术后系统化整体护理效果观察

摘　　要：目的探讨系统化整体护理在腹腔镜下完全腹膜外腹股沟疝修补术后的护理效果。方法对2013年2-7月30例住院患者进行常规护理，为实施系统化整体护理模式前；对2013年8月-2014年1月30例患者进行系统化整体护理。分析实施前后患者尿潴留、便秘出血水肿、疼痛发生率，引流管通畅率，术后进食、下床活动、住院时间等。结果实施系统化整体护理后患者尿潴留、便秘、出血水肿、疼痛发生率分别为0％，3．3％，3．3％，6．7％，较实施前（16．7％，20．0％，20．0％，33．3％）均明显下降，引流管通畅率由80％上升至100％，优于实施前，差异均有统计学意义（X^2值分别为5．455，4．043，4．043，6．670，4．043；P〈0．05）；实施系统化整体护理后患者术后进食时间、下床活动时间、平均住院日分别为（6．4±2．1），（6．8±3．1），（3．7±1．8）d均较实施前（9．2±2．6），（15．6±3．8），（6．1±2．2）d明显下降，比较差异有统计学意义（t值分别为4．589，9．829，4．625；P〈0．01）。结论腹腔镜下完全腹膜外腹股沟疝修补术后采用系统化整体护理效果良好，患者满意度比较高。Objective To investigate the effect of systematic approach to holistic nursing care mode ( SAHNCM ) after totally extraperitoneal inguinal hernia repair surgery under laparoscopic ( TEIHRSL ) . Methods From February to July 2013 for the implementation of SAHNCM, a total of 30 patients with TEPLIHRS received routine nursing;and from 2013 August to 2014 January, another 30 patients with TEIHRSL were taken SAHNCM. We analyzed the implementation of patients with urinary retention rate, incidence of constipation, the drainage tube unimpeded rate, postoperative eating time, ambulation time, hospitalization time. Results After the implementation of SAHNCM, the patients with urinary retention rate, incidence of constipation, rate of hemorrhage and edema, pain occurrence were significantly decreased to 0%, 3. 3%, 3. 3%, 6. 7% comparing with 16. 7%, 20. 0%, 20. 0%, 33. 3% before implementation; the drainage tube unimpeded rate increased from 80% to 100% (χ2 =5. 455, 4. 043, 4. 043, 4. 043, 6. 670,respectively;P<0. 05). At the same time, the patients′ postoperative eating time, ambulation time, hospitalization time were&nbsp;(6.4 ±2.1), (6.8 ±3.1), (3.7 ±1.8)d compared with (9.2 ±2.6), (15.6 ±3.8), (6.1 ±2.2) d of before SAHNCM implementation (t=4. 589, 9. 829, 4. 625;P<0. 01). Conclusions TEIHRSL works well with SAHNCM, which leads to a high rate of patient satisfaction.
