腰椎内固定术后尿管拔除最佳时间的临床观察

摘　　要：目的:观察全身麻醉下腰椎内固定术后留置尿管的拔除最佳时间。方法:将90例全麻腰椎术后患者按拔尿管时间分为三组。甲组30例,术后第1天患者自感有尿意时拔除尿管;乙组30例,术后第2天拔除尿管;丙组30例,术后第3天拔除尿管,观察三组患者在拔除尿管后排尿顺畅情况、尿路刺激征等指标。结果:乙组患者在术后顺利排尿率、尿路刺激征方面均优于其余各组,差异具有统计学意义(P<0.05)。结论:腰椎内固定术后第2天拔除尿管可以提高排尿成功率、减少尿潴留发生、降低尿路感染,有利于患者术后康复。Objective To observe the time of removals of postoperative lower lumbar internal fixation indwelling catheters under general anesthesia.Method 90 cases of postoperative patients of lower lumbar internal fixation under general anesthesia were divided into 3 groups by the time of catheter removal.Group A （30 cases）：the indwelling catheter was removed on the first postoperative day when the patient sensed a desire to urinate;Group B （30 cases）：the indwelling catheter was removed on the second postoperative day;Group C （30 cases）：the indwelling catheter was removed on the third postoperative day.Such indicators of the three groups of patients as the smoothness of urination and urination tract irritation of the patients in the three groups were observed.Result The smooth urination rates,urination tract irritation of patients of Group B were all superior to those of the other groups （P 〈0.05）.Con-clusion The postoperative second day is an effective nursing method for ensuring the success rate of urination,reducing the occurrences of urinary retention and lowering the rates of the urinary tract infections;meanwhile,it is beneficial to the patients＇post -operative re-covery.
