摘　　要：目的：探究布比卡因与罗哌卡因腰硬联合麻醉用于下肢骨关节手术围术期的有效护理方式。方法选取2012年～2014年我院行下肢骨关节手术病患681例为研究对象，所有病患均使用布比卡因与罗哌卡因腰硬联合的方式进行麻醉，现将所有病患随机分成护理组（341例）和对照组（340例）。每组病患相关医学参数无统计学意义存在，具有可比性。对照组使用常规方式进行围手术期麻醉护理。治疗组依照病患的相关情况，对其进行个性化麻醉护理。将两组病患麻醉前（T0），麻醉后6h（T1），麻醉后24h的静脉凝血时间，以及围手术期相关情况进行对比。结果护理组病患围手术期和麻醉前（T0），麻醉后6h（T1），麻醉后24h的静脉凝血情况优于对照组，且 P＜0．05。结论利用腰硬联合麻醉的方式，能够有效预防下肢骨关节手术病患围手术期血液高凝状态，护理人员在围手术期内对其进行有效护理，能够起到满意效果，对病患康复具有促进作用。OBJECTIVE To explore the bupivacaine and ropivacaine in combined spinal epidural anesthesia for lower limb bone and joint operation effective nursing methods in perioperative period.METHODS In our hospital from 2012 to 2014 681 cases of lower limb bone joint surgery were selected as the Research object.All patients were using bupivacaine and ropivacaine for cambined spinal epidural anesthesia.patients were randomly divided into nurs-ing group （341） and control group （340）.The medical parameter of two groups has no statistical significance.The control group using conventional methods of perioperation period of anesthesia nursing.The treatment group carries on the personalized anesthesia nursing.the time before anesthesia （ T0 ） ,6 hours after anesthesia （ T1 ） ,venous blood co-agulation of two time of 24 hours after anesthesia,and the peri operation period and related condition of two groups of were compared.RESULTS The nursing of patients during perioperative period and before anesthesia （ T0 ） ,6 hours after anesthesia （ T1 ） ,24 hours after anesthesia and Venous blood coagulation was better than that of control group（ P〈0.05）.CONCLUSION The use of combined spinal epidural anesthesia effective prevent the lower limb bone and joint surgery patients perative blood hypercoagulabble state,effective nursing in the perioperation period can play a satisfactory effect.
