抗生素灌洗疗法联合高压氧治疗骨髓炎的临床观察

摘　　要：目的观察抗生素灌洗疗法联合高压氧治疗骨髓炎的临床治疗效果。方法将62例慢性骨髓炎患者随机分为研究组和对照组,每组31例。所有患者均反复清创,大量抗生素生理盐水灌注冲洗引流,同时进行长期抗生素控制。研究组在此基础上应用高压氧治疗。比较两组患者的治愈率、复发率、细菌检出率、抗生素应用时间、愈合时间、住院时间和炎症因子水平。结果研究组治愈率显著高于对照组(p<0.01),且复发率显著低于对照组(p<0.05)。治疗后,研究组患者细菌检出率显著低于对照组(p<0.05)。研究组抗生素应用时间、愈合时间和住院时间显著低于对照组(p<0.01)。治疗前两组患者骨髓组织中的IL-6和TNF-α水平无统计学差异(p>0.05),治疗后研究组患者的IL-6和TNF-α水平均显著低于对照组(p<0.01)。结论抗生素灌洗疗法联合高压氧治疗骨髓炎的临床疗效显著,具有临床应用价值。Objective To evaluate the clinical curative effect of antibiotic irrigation combined with hyperbaric oxygen therapy in the treatment of osteomyelitis. Methods 62 patients with osteomyelitis were randomly divided into experiment group and control group with 31 in each. All the patients were treated with repeated debridement,antibiotic irrigation and long- term antibiotic therapy. The patients in the experiment group were treated with hyperbaric oxygen therapy additionally. The two groups were compared in terms of curative rate,recurrence rate,bacteria detection rate,antibiotic usage duration,healing time,in- hospital time and inflammatory factor level. Results The curative rate in the experiment group was significantly higher than that of the control group（ p 〈 0. 01）,and the recurrence rate was significantly lower（ p 〈 0. 05）. The bacteria detection rate in the experiment group was significantly lower than that of the control group,after treatment（ p 〈 0. 05）. The antibiotic usage duration,healing time and in-hospital time in the experiment group were significantly lower than that of the control group（ p 〈 0. 01）. There was no significant difference of IL- 6 and TNF- α between two groups before treatment（ p 〉 0. 05）. After treatment,both of the IL- 6 and TNF- αin in the experiment group were significantly less than those of the control group（ p 〈 0. 01）.Conclusion Antibiotic irrigation combined with hyperbaric oxygen therapy has a great clinical efficacy on osteomyelitis,and therefore is worthy of clinical application.
