血清解整合素-金属蛋白质酶12-S在异位妊娠早期诊断中的价值探讨

摘　　要：目的观察解整合素-金属蛋白质酶12-S(ADAM12-S)在异位妊娠及正常妊娠过程中的变化特点,探讨ADAM12-S在异位妊娠早期诊断中的临床价值。方法将146例异位妊娠患者设为异位妊娠组,263例正常孕妇为对照组,观察两组研究对象不同孕周的ADAM12-S检测情况,以及ADAM12-S与孕周相关性。结果异位妊娠组平均ADAM12-S对数值在不同孕周均低于同期正常妊娠组,差异有统计学意义(P<0.05);正常对照组ADAM12-S对数值与孕周呈线性关系(r=0.926,P=0.000,P<0.01),回归方程Y=0.3147X-0.3099,ADAM12-S对数值随孕周增加而上升;异位妊娠组ADAM12-S对数值与孕周经直线相关回归性分析,无线性关系(r=-0.119,P=0.140,P>0.05)。结论 ADAM12-S在妊娠早期阶段能较好地反映胎盘功能情况,可作为异位妊娠诊断的辅助检查指标。Objective To observe a disintegrin and metalloprotease（ADAM）12 - S changes in ectopic and normal pregnancy,and to explore the clinical value of ADAM12 - S in the early diagnosis of ectopic pregnancy. Methods One hundred forty - six cases of ectopic pregnan-cy were set as ectopic pregnancy group,263 normal pregnant women as control group. ADAM12 - S was detected in two groups of subjects in dif-ferent pregnant weeks and the relationship between ADAM12 - S and gestational weeks was analyzed. Results Average ADAM12 - S values at different gestational age for ectopic pregnancy group were lower than the same period in normal pregnancy group,the difference was statistically sig-nificant（ P 〈 0. 05）. A positive linear correlation was found between the logarithm of ADAM12 - S and gestational age（ r = 0. 926,P = 0. 000, P 〈 0. 01）in the control group（the regression equation was y = 0. 3147X - 0. 3099）. No linear relationship was found between the logarithm of ADAM12 - S and gestational age in ectopic pregnancy group（ r = - 0. 119,P = 0. 140,P 〉 0. 05）. Conclusion ADAM12 - S can reflect pla-cental function in the early stages of pregnancy and can be used as an auxiliary indicator for the diagnosis of ectopic pregnancy.
