彩色多普勒超声在脾动脉瘤诊断中的应用价值

摘　　要：目的探讨超声彩色多普勒血流成像对脾动脉瘤(SAA)的诊断价值。方法回顾性分析13例脾动脉瘤患者的超声表现及彩色多普勒血流成像(CDFI)图像特征,所有患者均经手术及病理证实。结果 13例SAA患者中,单发11例,多发2例,2例患者2个瘤体,共15个SAA;超声误诊2个,准确诊断13个,诊断准确率为86.7%(13/15)。SAA的超声特征性表现为:脾动脉管径局限性扩张,瘤体内充满彩色血流信号,与脾动脉相通。结论超声能方便、准确、无创地显示SAA及瘤体内的血流动力学改变,适合临床对SAA进行早期诊断。Objective To study the value of color Doppler ultrasound in diagnosing splenic artery aneurysms（SAA）. Methods Thirteen patients with surgically and pathologically proved splenic artery aneurysm underwent preoperative ultrasonography examination. Results Thirteen cases with 15 aneurysm：11 cases were solitary,and 2 cases were multiple,2 cases had 2 aneurysms . Two cases was misdiagnosed by ultrasound. 13 of the 15 aneurysms were diagnosed by CDFI,and the diagnosis accuracy was 86. 7%（13 / 15）. The feature appearances on CDFI in SAA were local expansion on the splenic artery,and colorful flow shown inside the aneurysm,which communicated with the splenic artery. Conclusion Ultrasonography can reveal reliably the SAA and the changes of blood flow. CDFI is suitable for clinical preliminary diagnosing and dynamic monitoring of the SAA.
