摘 要:目的分析我院实行处方点评制度前后门诊处方质量的变化以及存在的一些问题,介绍先进的管理理念,促进临床药物的合理应用。方法采用回顾性分析方法,总结分析2012年1月至2013年6月的处方点评分析记录。针对结果提出质量改进建议,总结先进管理经验,制定合理用药的改进措施。结果在随机抽取的20 400张处方中,经药师仔细点评,不合理处方有434张,占总抽样处方量的2.13%。错误类型分布为:诊断与用药不符占30%、用法用量不合理占26%、给药途径不合理占6%、超量占34%、抗菌药物超量占4%。结论通过处方点评,门诊处方合格率有较大的提高,使得错误处方的数量不断减少,最终实现集中解决核心问题,达到了质量持续改进的目的,但在点评过程中仍存在问题。Objective To analyze the prescription changes in the quality and the problems before and after the implementation of prescription review system. To introduce advanced management concepts and promote rational clini-cal use of drugs. Methods The prescription comment records from January 2012 to May 2013 were summed up and analyzed by retrospective analysis methods. Quality improvement suggestions were proposed based on results. The ad-vanced management experience was summarized and improvement measures of rational drug use were made. Results Pharmacists carefully reviewed a random sample of 20 400 prescriptions which had 434 irrational prescriptions,account-ing for 2. 13% of the total. Among of these irrational prescriptions,there were 30% diagnosis and medication discrepan-cies,26% unreasonable usage and dosage,6% inappropriate administration route,34% extraordinary prescriptions and 4% extraordinary antibacterial. Conclusion By prescription comment,outpatient prescription pass rate has greatly im-proved and achieve the purpose of continuous improvement of quality,but the problems still exist in the review process.
