地特胰岛素联合个体化延伸护理在超重2型糖尿病起始胰岛素治疗中的疗效

摘　　要：目的探讨和分析地特胰岛素联合个体化延伸护理治疗超重2型糖尿病患者的疗效,观察其对患者体重的影响。方法选取68例口服药物治疗血糖仍控制不佳的超重2型糖尿病患者,给予地特胰岛素治疗并配合系统健康教育及个体化延伸护理12周,观察患者治疗前后的体重指数(BMI)、空腹血糖(FPG)、餐后2 h血糖(2h PG)、糖化血红蛋白(Hb Alc)、胰岛素使用剂量及有无低血糖等不良事件发生。结果治疗后,患者FPG、2h PG、Hb A1c明显下降(P<0.01),BM I水平也有所下降(P<0.05)。结论地特胰岛素联合个体化延伸护理可以有效控制2型糖尿病患者的血糖,同时降低患者体重,是超重2型糖尿病患者应用胰岛素治疗的理想选择。Objective To investigate the effect of insulin detemir combined with individual extended care on the overweight patients with type 2 diabetes mellitus,and to observe the effect on the patient′s weight control. Methods Sixty overweight patients with type 2 diabetes whose blood glucose were still under poor control based on the oral medication were selected,they were treated with insulin detemir combined with systematic health education and individ-ual extended care for 12 weeks,the patient＆#39;s body mass index （BMI）,fasting blood glucose （FPG）,2 h postprandial blood glucose （2hPG）,glycosylated hemoglobin （HbAlc）,insulin dose and the adverse events of hypoglycemia were observed before and after treatment. Results After treatment,FPG,2hPG,HbA1c of all the patients decreased signifi-cantly （P〈0. 01）,BMI levels also decreased （P〈0. 05）. Conclusion Detemir combined with individual extended care can effectively control the blood sugar of patients with type 2 diabetes,reduce the patient′s weight,and it is the ide-al choice for the overweight type 2 diabetes mellitus patients.
