笑气吸入和丙泊酚静脉无痛胃镜检查的效果比较

摘　　要：目的比较笑气吸入镇痛和丙泊酚静脉全麻在胃镜诊治中的镇痛效果,总结无痛胃镜检查与治疗过程中的护理。方法接受无痛胃镜诊疗的患者112例,随机分为笑气镇痛组和丙泊酚组,各56例。笑气镇痛组及丙泊酚组分别由护理人员和麻醉医生操作。监测患者生命体征,观察记录镇痛效果、并发症、患者满意率等。结果患者均顺利完成检查与治疗,丙泊酚组心动过缓和呼吸抑制的发生率高于笑气镇痛组(P<0.05),笑气镇痛组胃镜检查时恶心、呕吐、呛咳的发生率高于丙泊酚组(P<0.05),丙泊酚组总有效率和患者满意率高于笑气镇痛组(P<0.01)。结论笑气吸入镇痛和丙泊酚静脉镇痛均有效缓解患者对胃镜诊治的恐惧和术中疼痛,笑气吸入在胃镜操作中消除患者不适的有效性较差,但更安全,对年老体弱者更适用,积极的护理对提高笑气吸入镇痛效果更为关键。Objective to compare the nitrous oxide inhalation analgesia with propofol intravenous anesthesia analgesic effect in the diagnosis and treatment of gastroscope ,summarizes the painless gastroscopy and the treatment process of nurs‐ing .methods Nitrous oxide analgesia with propofol intravenous anesthesia group all 56 cases ,anesthesia and analgesia and anesthesia group respectively by the nursing staff doctor .Monitoring the patient vital signs ,observation records analgesic effect ,complications ,and patient satisfaction .Results the patients were successfully completed examination and treatment , but the propofol group bradycardia and the incidence of respiratory depression is higher than the nitrous oxide group （P〈0 .05） ,nitrous oxide group the incidence of nausea ,choking cough when gastroscopy is higher than the propofol group （P〈0 .05） ,the overall efficiency and patient satisfaction propofol group was higher （P〈0 .05） .Conclusion nitrous oxide inhala‐tion and intravenous propofol are effective in relieving patients fear of diagnosis and treatment of gastroscope and intraopera‐tive pain ,laughing gas suction in gastroscope operation to eliminate the effectiveness of patient discomfort but poor more se‐cure ,more applicable to frail elderly ,active care to improve the effect of nitrous oxide inhalation analgesia is more critical .
