腰围/身高比值与2型糖尿病肾病的相关性研究

摘　　要：目的探讨腰围及腰围/身高比值(WHt R)与糖脂代谢和胰岛素相关指数的关系,以及对2型糖尿病肾病的影响。方法 2013年11月至2014年4月收治的95例2型糖尿病住院患者,按照WHt R>0.5或WHt R≤0.5分为两组,分别检测并比较两组患者肝肾功能、血脂、血清胱抑素C(Cys C)、空腹血糖(FPG)及餐后2小时血糖(2h PG)、空腹C肽(FCP)、胰岛素抵抗指数(HOMA-IR)、胰岛素功能指数(HOMA-IS)、24小时尿微量清蛋白(24h UALB)、体质量指数(BMI)、腰围(WC)、臀围(HC)、腰围/臀围(WHR)等的差异,分析WHt R、Cys C与肝肾功能指标[丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、24h UALB]、血脂[三酰甘油(TG)]、FPG、2h PG、FCP、HOMA-IR、HOMA-IS、BMI、WHR、WC及HC的关系。结果 (1)WHt R>0.5组WHR、WC、HC、2h PG、FCP、HOMA-IR、BMI、24h UALB、TG及Cys C明显高于WHt R≤0.5组,差异均有统计学意义(P<0.05)。(2)Pearson相关性分析显示,2型糖尿病患者的WHt R与HC、WC、WHR、BMI、HOMA-IR、HOMA-IS、Cys C、FCP呈正相关(P<0.05),Cys C与HOMA-IS、血肌酐(SCr)、尿酸(UA)、24h UALB呈正相关(P<0.01)。结论WHt R对糖脂代谢和2型糖尿病患者胰岛B细胞功能及胰岛素抵抗的判断有辅助参考价值;WHt R与Cys C、FCP、HC、WC、BMI联合测定可作为预测糖尿病肾病的危险信号及肾功能的早期指标。Objective To discuss the relationship between waist circumference/height ratio （WHtR） with glucolipid metabolism and insulin related indexes, and the effect on type 2 diabetic nephropathy. Methods Divided 95 type 2 diabetic nephropathy inpatients into WHtR〉0.5 group and WHtR ≤0.5 group. Tested and compared two groups＇ liver function ,renal func tion, blood fat, cystatin C （ CysC ）, fasting blood glucose （FPG）, 2h postprandial blood glucose （2hPG）, fasting C -peptide （ FCP ）, in- sulin resistance index （ HOMA-IR ）, insulin function index （HOMA-IS）, 24h urine albumin （24hUALB）, body mass index （BMI）, waist circumference （WC）, hip circumference （ HC ）, waist circumference/hip circumference （WHR）, and age differences. Ana- lyzed the relationship among WHtR, CysC and liver function, renal function, blood fat, FBG, 2hPBG, FCP, 24hUALB, HOMA-IR, HOMA-IS,BMI,WHR,WC,HC. Results （1）WHtR〉0.5 group＇s WHR,WC,HC,2HPBG,FCP,HOMA-IR,BMI,24hUALB,tri acylglycerol （TG） and CysC were clearly higher than WHtR ~〈 0.5 group＇ s, differences showing statistical significance （P〈0.05）. （2） Pearson correlation analysis showed that type 2 diabetic nephropathy inpatients＇WHtR was positively correlated with HC, WC, WHR, BMI, HOMA-IR, HOMA-IS, CysC and FCP（P〈0.05 ）, and their CysC was positively correlated with HOMA-IS, creatinine, trioxypurine and 24hUALB（P〈0.01 ）. Conclusion WHtR has reference value in judging glucolipid metabolism, type 2 diabetic nephropathy inpatients＇ islet B cells＇ function and insulin resistance. Simultaneously determing WHtR and CysC, FCP, HC, WC, BMI can be used as the risk signal to predict diabetic nephropathy and renal function＇s early stage index.
