温灸肩井穴预防甲状腺手术体位综合征60例临床观察

摘　　要：目的观察温灸肩井穴预防甲状腺手术体位综合征的临床疗效。方法将120例甲状腺手术患者随机分为治疗组和对照组各60例。两组患者均给予规范的术前常规护理和体位训练,治疗组于术前2日体位训练后及手术后麻醉完全清醒后温灸每日1次,共5天。记录两组患者的手术时间并比较两组患者头晕头痛、恶心呕吐、颈肩痛的发生情况。结果治疗组中手术时间≤1h、1~2h、≥2h的患者分别为8例、13例和39例,对照组分别为11例、9例和40例,两组患者手术时间分布比较,差异无统计学意义（P〉0.05）。治疗组手术后头晕头痛（5例）、恶心呕吐（3例）、颈肩痛（4例）发生例数明显少于对照组的21例、16例、25例,差异有统计学意义（P〈0.01）。结论温灸肩井穴能有效预防甲状腺患者术后手术体位综合征的发生。To observe clinical effects of warm moxibustion at Jianjing （ GB21 ） for preventing thyroid operative position syndrome. Methods Totally 120 patients undergoing thyroid surgery were randomly divided into treatment group and control group, with 60 patients in each group. All the patients were given preoperative routine nursing and posture training. After posture training two days before operation and after postoperation fully awake from anesthesia, warm moxibustion was given to the treatment group once per day, for 5 days. Recorded operation time of patients in both groups and compared the occurrence of dizziness and headache, nausea and vomiting, neck and shoulder pain in the two groups. Results In the treatment group, patients with operation time~〈lh, 1 -2h, ~〉2h were 8 cases, 13 cases and 39 cases respectively, however, those the control group were 11 cases, 9 cases and 40 cases respectively. Operation time distribution in patients of both groups has no statistically significant difference （P 〉 0. 05 ）. After the operation, in the treatment group, cases of dizziness and headache （5 cases）, nausea and vomiting （3 cases）, neck and shoulder pain （4 cases） were significantly less than those in the control group （with 21 cases, 16 cases, 25 cases respectively）. There was statistically significant difference between the two groups （P 〈 0. 01 ）. Conclusion Warm moxibustion at Jianjing （ GB21 ） can effectively prevent the development of thyroid operative oosition syndrome.
