铝镁加联合兰索拉唑治疗重型颅脑损伤应激性溃疡的临床研究

摘　　要：目的评价铝镁加联合兰索拉唑治疗重型颅脑损伤应激性溃疡的临床疗效及安全性。方法 60例重型颅脑损伤应激性溃疡患者随机分为试验组28例和对照组32例。2组均实施常规对症治疗,给予脱水、激素等积极性治疗,维持电解质及酸碱平衡,并给予适当的营养支持。对照组静脉滴注兰索拉唑30 mg+0.9%氯化钠注射液100 m L,每日2次;试验组在对照组治疗的基础上加用铝镁加1.5 g(15 m L),鼻饲,每日3次,2组疗程均为7 d。观察2组患者临床疗效、胃酸变化及服药期间不良反应发生率。结果试验组总有效率为82.1%,对照组为71.8%,2组差异无统计学意义(P>0.05)。治疗后,2组患者胃酸p H值均较治疗前显著升高(P<0.05);治疗后6 h开始,试验组胃酸p H显著高于对照组(P<0.05)。2组用药期间未见明显不良反应。结论铝镁加联合兰索拉唑治疗重型颅脑损伤应激性溃疡可迅速升高患者胃酸p H值,临床疗效确切,无明显不良反应。Objective To evaluate the clinical efficacy and safety of almagate combined with lansoprazole in the treatment of stress ulcer patients with heavy craniocerebral injury .Methods A total of 60 stress ulcer patients with heavy craniocerebral injury were included in this study and were divided into experiment group （ n=28 ） and control group （ n=32）.Patients in two groups all received regular treatment and nutrition such as dehydration and hormone to maintain electrolyte and acid -base balance.The patients in control group were treated with lansoprazole 30 mg＋0.9%NaCl 100 mL by intravenous drip.Patients in the experiment group were given lansoprazole and almagate 1.5 mL by nasal feeding , tid.The course of the two groups were 7 days.The data of total efficacy , gastric acid pH and adverse drug reactions were compared between the two groups.Results The total efficacy were 82.1%and 71.8%in the experiment and control group , respectively , which showed no statistical difference （ P〉0.05 ） . The pH of gastric acid after treatment was significantly higer than that before treatment （ P 〈0.05 ） .The pH of gastric acid was significantly different 6 hours after treatment between the two groups （ P〈0.05 ） .No obvious adverse drug reaction was observed in the two groups in the period of treatment .Conclusion Almagatecombined with lansoprazole is effective and safe in the treatment of stress ulcer patients with heavy craniocerebral injury.
