患者和献血员Rh血型表型的检测与临床输血

摘　　要：目的研究患者和献血员Rh血型表型(即E、e、C、c抗原)的检测在临床输血中的意义,指导临床安全、合理、有效用血。方法采用微柱凝胶法常规检测患者和献血员的Rh表型,对Rh阴性患者血样使用3份不同批号的抗人球蛋白试剂,采用试管法进行Rh(D)阴性确认试验,排除弱D或不完全D。利用计算机建立相应的Rh表型库,在临床输血过程中,选用与患者表型相同或相近的的献血员悬浮红细胞进行交叉配血。结果本组患者血样中共检出Rh(D)阴性个体62例,检出率为0.32%(62/19 252)。从表型数据库中可以发现患者和献血员Rh表型出现的频率基本一致:CCDee>Cc DEe>Cc Dee>cc DEE>cc DEe>CCDEe>Cc DEE>cc Dee>CCDEE。结论建立Rh血型表型库,选用与患者Rh表型相同或相近的的悬浮红细胞输注,可减少Rh抗体的产生,对保障安全、合理、有效的临床用血具有重要的临床意义。Objective ＂ To study the Pall phen^typeln patients and donors （that is E ,e,C ,c Antigen; of the significance in clin- ical blood transfusion, Guide the clinical department safety, reasonable, effective use of blood. Methods Rh phenotype in pa- tients and blood donors was routine tested by mieroeolumn gel method, Patients with Rh（D） negative blood samples using three different batch test tube method for the anti - human globulin reagent for Rh （D） - negative confirmation test to exclude the weak D or incomplete D. Using computer to establish the appropriate lth phenotype library, In the course of clinical blood trans- fusion, selection of patients with the same or similar phenotypes donors suspended erythroeytes were cross matched. Results This group of patients blood samples were detected in the Rh （D） negative individuals in 62 eases, the detection rate was 0.32% （62/19 252）. From table database it can be found the frequency of Rh phenotypes in patients and blood donors were consistent, which was CCDee 〉 CcDEe 〉 CeDee 〉 ceDEE 〉 ecDEe 〉 CCDEe 〉 CcDEE 〉 ccDee 〉 CCDEE. Conclu- sion The establishment of Rh blood phenotype library, selection of patients with the same or similar suspension of Rla pheno- type RBCs infusion may reduce the production of Rh antibodies and guarantee safety, reasonable and effective clinical use of blood is of great clinical significance.
