改良TLIF技术在高位腰椎间盘突出症治疗中的应用

摘　　要：目的 评价改良TLIF技术治疗高位腰椎间盘突出症的临床价值。方法 2009年8月至2013年8月,采用改良TLIF技术手术治疗27例高位腰椎间盘突出症患者,其中男19例,女8例;年龄34-56岁,平均（44.93±6.29）岁;病变节段：L1-25例,L2-38例,L3-414例。术前、术后分别采用VAS法进行评分,测量椎间隙相对高度,评价临床结果。结果 本组患者均获得随访,随访时间12-18个月（平均14.5个月）。术后末次随访VAS评分较术前均得到明显改善,椎间融合率为100%。无脊髓、神经根损伤,无切口感染,无脊柱感染,无严重并发症。结论 改良TLIF技术是一种安全、有效的治疗高位腰椎间盘突出症的方法,值得临床推广。Objective To evaluate the clinical value of improved TLIF technique in the treatment of high lumbar disc herniation. Methods In August 2009 to August 2013,27 patients of high lumbar disc herniation were treated with Improved TLIF technique, and followed up for 12 - 18 months （ mean 14.5 months）. Preoperative and postoperative VAS score and intervertebral relative height were used to evaluate the clinical results. Results The last follow-up VAS score was obviously improved compared with the before operation,intervertebral fusion rate was 100%. Without spinal cord and nerve root injury, infection of incision, no spinal infection, no serious complications were observed. Conclusion TLIF technique is a safe and effective method for treatment of high level lumbar disc protrusion, and worth more clinical promotion.
