网络医疗健康管理在肿瘤化疗间歇期PICC带管患者中的应用研究

摘　　要：目的探讨对肿瘤化疗间歇期PICC带管患者实施健康管理的效果。方法选取新疆维吾尔自治区人民医院肿瘤化疗PICC患者138例,按照随机数字表将患者分为观察组和对照组各69例。观察组采用网络医疗健康管理方式进行干预,对照组按照传统的健康教育模式进行电话随访。3个月后观察两组患者导管不良事件、并发症、非计划性拔管发生率和导管自我管理能力。结果两组患者自身前后及两组之间导管自我管理能力总分及各维度得分差异有统计学意义(P<0.05)。两组患者血栓发生率比较差异无统计学意义(P>0.05);观察组患者静脉炎、导管感染、导管堵塞、导管断裂、导管脱出率和非计划性拔管率均低于对照组,差异有统计学意义(P<0.05)。结论对肿瘤化疗间歇期PICC带管患者实施健康管理,显著提高了患者PICC导管自我管理能力,基于网络医疗平台科学规范的健康管理更进一步降低了患者导管不良事件、并发症和非计划性拔管的发生率。Objective To discuss the effect of health management on patient with PICC tube during Intermittentchemotherapy. Methods We had selected 138 cases of patient with PICC tube during tumor chemotherapy in People＇s Hospital of Xinjiang Uygur Autonomous Region randomly and patients were divided into experiment group and control group. In experiment group, network medical health management mode was implemented for health intervention and in control group, traditional health education mode of telephone follow-up was used. The incidence of complications, rate of unplanned extubation and patients＇ self-management ability of catheter were recorded three months after health management. Results The difference was statistically significant in self- management ability of catheter in two groups （P〈0.05）. There was no statistically significant difference on the incidence of thrombosis （P〉0.05） ;the incidence of phlebitis, catheter infections, catheter jam, catheter fracture, the falling off of catheter and unscheduled decannulation in experiment group was obviously lower than that in control group and the difference was statistically significant（P〈0.05 ）. Conclusion The implementation of health management based on network medical platform for patient with PICC tube during intermittent chemotherapy can significantly reduce catheter-related complications, adverse events of patients and unplanned extubation rate, and improve patients＇ self-management ability of catheter.
