肾病血液透析患者中心静脉导管血栓及纤维鞘形成的护理研究

摘　　要：目的:探讨干预护理对肾病血液透析患者中心静脉导管血栓、纤维鞘形成及并发症发生的临床效果。方法:选取肾病血液透析中心静脉置管的患者64例,随机分为对照组和研究组,每组各32例。对照组以常规护理进行干预,研究组在对照组的基础上采取针对性护理干预。比较两组患者血栓及纤维鞘的形成和再形成率以及并发症情况。结果:对照组的血栓及纤维鞘的形成和再形成率分别为78.13%和62.50%,研究组的血栓及纤维鞘的形成和再形成率分别为43.75%和28.13%,研究组形成率要低于对照组,差异具有统计学意义(P<0.01);研究组的并发症情况明显低于对照组,差异具有统计学意义(P<0.01)。结论:对肾病血液透析中心静脉置管的患者进行针对性干预护理能有效降低导管血栓及纤维鞘的形成率、减少并发症的发病率,值得临床推广。Objective To investigate the clinical effects of nursing interventions on formation of central venous catheter thrombosis and fi- brous sheath and complications of Kidney dialysis patients. Method 64 cases of kidney hemodialysis patients in kidney dialysis centers were selected and randomly divided into control group and study groups ,each group 32 cases. Patients in control group were given routine nursing intervention, and patients in study group were given targeted nursing intervention based on control group. The formation and re - formation rate of thrombosis and fibrous sheath and complications of two groups were compared. Results The formation and re - formation rate of thrombosis and fibrous sheath of control group were 78. 13% and 62. 50% ,respectively, and fomlation and re -formation rate of thrombosis and fibrous sheath of study group were 43.75% and 28. 13 % ,respectively, the formation rate of study group was lower than control group, the difference was statistically significant （P 〈 0. 01 ） ;The complications of study group was significantly lower than control group, the difference was statisti- cally significant （P 〈 O. 01 ）. Conclusion For kidney hemodialysis patients in kidney dialysis centers, targeted nursing intervention can effec- tively reduce the formation rate of thrombosis and fibrous sheath, and reduce the incidence of complications, worthy of promotion.
