摘 要:目的探讨人类白细胞抗原(Human Leukocyte Antigens,HLA)DP基因多态性与高危人群丙型肝炎病毒(Hepatitis C Virus,HCV)感染转归的关系。方法采用病例对照研究设计,运用Taqman-MGB技术检测401例HCV持续感染者,268例HCV自限感染者和984例健康对照者HLA-DP基因rs3077、rs9277535、rs2395309位点的基因多态性,比较不同基因型与HCV感染转归的关系。结果采用Logistic回归分析调整性别、年龄和高危人群种类混杂因素后结果显示,rs3077位点:杂合基因型CT和突变型TT在感染组(持续感染组+自限清除组)中的频率均显著高于对照组(调整OR值及其95%CI分别为1.31,1.05~1.64和1.93,1.36~2.73);rs2395309位点:杂合基因型GA和突变型AA在感染组中的频率均显著高于对照组(调整OR值及其95%CI分别为1.29,1.03~1.61和1.89,1.34~2.68);rs9277535位点:杂合基因型GA和突变型AA在持续性感染组中的频率均高于自限清除组(P=0.018,P=0.037)。结论rs3077、rs9277535、rs2395309三个位点的基因多态性可能与丙型肝炎转归有一定的关联。Objective To assess the relationship between Human leukocyte antigens (HLA) DP gene polymorphisms and the susceptibility or the outcome of HCV infection among high-risk population in Jiangsu Province. Methods Three HLA-DP variants of rs3077, rs9277535 and rs2395309 were genotyped in 1653 subjects, including 268 self-limited HCV persistent infections, 401 persistent HCV infections and 984 healthy controls by using the Taqman- MGB technology. Results In the comparison of HCV infected subjects (HCV persistent carriers and HCV natural clearance subjects) and uninfected subjects, the genotypes CT and rFT of rs3077 increased the risk of HCV infection (OR=1.31, 95% CI: 2.05-2.64; OR=1.93, 95% CI: 1.36-2.24 ), and the frequency of rs2395309 mutant genotype AA as well as heterozygous genotype GA in HCV infected group were much higher than that in the control group (OR=1.29, 95% CI: 1.03-1.61; OR=1.89, 95% CI: 1.34-2.68 ). Besides, compared with HCV natural clearance subjects carrying rs9277535, those with GA and AA genotypes had an increased risk (P=0.018, P=-0.037). Conclusion These variants in the HLA-DP locus can be associated with the outcomes of HCV infection in high-risk Chinese population.
