高龄老人心力衰竭209例临床特点及用药现状分析

摘　　要：目的分析高龄老人心力衰竭的临床特点及用药现状。方法收集2011年6月至2014年6月209例年龄≥80岁的高龄心力衰竭住院患者的临床资料,分析高龄老人心力衰竭的临床特点、病因、诱因、用药类别及药物的不良反应发生率。结果高龄老人心力衰竭的临床特点:多种慢性疾病并存,相互协同导致;临床表现多不典型;常需多专业综合治疗;预后与治疗的及时性和有效性密切相关;并发症多,死亡率高。高龄老人心力衰竭的病因依次为冠心病、高血压性心脏病、心律失常等;诱因依次为肺部感染、心律失常、心肌缺血等;用药类别包括利尿剂、血管扩张剂(硝酸酯类)、β受体阻滞剂等;药物的不良反应发生率为13.9%,主要为酸碱失衡及离子紊乱、低血压、心律失常等。结论高龄心力衰竭老人普遍有多种慢性疾病并存,相互协同导致心力衰竭。高龄老人心力衰竭有其独特的临床特点,治疗常需多学科综合治疗,治疗药物类别多,其药物不良反应发生率高。Objective To analyze the clinical characteristics and current situation of drug use in elderly patients with heart failure （HF）. Methods Totally 209 elderly in-patients （198 male patients and 11 female patients,whose average age was 85.94 ~ 5.56 years old） were enrolled from June 2011 to June 2014. We analyzed clinical characteristics,pathogeny,incentive of HF,drug cate- gory and incidence of drug adverse effect. Results Clinical characteristics in elderly patients with HF： Multiple chronic diseases coexist which cooperated to result in HF;Clinical manifestation was atypical;Comprehensive treatment of multi-subjects;Prognosis correlated closely with prompt and efficacy of therapy ;Much complications ,high mortality rate. Pathogeny was coronary heart dis- ease （CHD）,hypertensive heart disease and arrhythmia,etc. Incentive was pneumonia, arrhythmia and myocardial ischemia, etc. Drug category used ：diuretic agent ,nitrate, [^-blocker,etc. Incidence of drug adverse effect was 13.9% ：mainly in acid-base imbal- ance and ion disorder,hypotension,arrhythmia,etc. Conclusion The elderly patients with HF coexist with multiple chronic dis- eases which cooperated to result in HF,having distinct clinical characteristics,needing comprehensive treatment of multi-subjects. Many drug categories for HF should be used and the incidence of drug adverse effect is high.
