持续质量改进在ICU失禁相关性皮炎管理中的实践

摘　　要：[目的]探讨护理持续质量改进(CQI)对减少重症监护室(ICU)失禁相关性皮炎(IAD)发生率的效果。[方法]对2013年7月—2013年12月IAD病例进行分析,于2014年1月—2014年6月运用CQI程序进行质量改进,包括成立质量改进小组、分析发生原因、建立预期目标、开展循证培训,制订预防方案,改善护理流程等,评价CQI前后的效果。[结果]实施CQI后,IAD的总发生率从41.41%降至29.66%,差异有统计学意义(P<0.05),其中重度IAD的发生率从8.59%降至2.76%,差异有统计学意义(P<0.05)。[结论]实施CQI减少了IAD在ICU的发生、发展。Objective:To probe into the effect of continuous quality improvement (CQI)for reducing the inci-dence of incontinence associated dermatitis (IAD)in ICU.Methods:The IAD cases were analyzed from July to December in 2013,the CQI program was used for quality improvement from January June in 2014,including the establishment of quality improvement teams,analysis of the causes,establishment of desired objectives,car-rying out the evidence based training,and development of prevention programs,improving the nursing proces-ses,then to evaluate the effect before and after CQI.Results:After implementation of CQI,the total incidence of IAD was decreased significantly from 41.41% to 29.66% (P <0.05),and the incidence of mid severe IAD was decreased significantly from 8.59% to 2.76% (P <0.05).Conclusion:The implementation of CQI has reduced the occurrence and development of IAD in ICU.
