摘 要:目的:探讨二维联合四维超声检查诊断胎儿畸形的临床价值,为胎儿畸形的产前诊断提供理论依据。方法:选取2009年10月~2013年10月在该院检查的中晚期孕妇3 192名,先行二维超声检查,对可疑畸形胎儿再行二维超声联合四维超声检查,根据引产、分娩或到上级医院进一步检查结果对比分析单纯二维超声检查及联合超声检查对诊断胎儿畸形的诊断符合率。结果:3 192名中晚期孕妇共分娩3 225个胎儿,检查出畸形胎儿123个(畸形158处),经引产、分娩或到上级医院进一步检查随访证实畸形胎儿104个(畸形135处),失访畸形胎儿19个(畸形23处)。135处胎儿畸形中,联合超声诊断胎儿畸形符合率约91.1%(123处),明显高于单纯二维超声77.8%(105处),两种检查方法比较,差异有统计学意义(P〈0.05);经随访证实体表畸形72处,联合超声诊断符合率约91.6%(66处)明显高于单纯二维超声73.6%(53处),两种检查方法比较,差异有统计学意义(P〈0.05)。结论:二维超声检查是诊断胎儿畸形的首选方法,四维超声检查诊断胎儿体表畸形具有明显优势,两者联合应用可显著提高胎儿畸形的诊断符合率,具有较高的临床应用价值。Objective: To explore the chnical value of two-dimensional ultrasanography combined with four-dimensional ultrasonography in diagnosis of fetal malformations, provide scientific basis for prenatal diagnosis of fetal malformations. Methods: A total of 3 192 middle and late pregnant women receiving prenatal examination in the hospital from October 2009 to October 2013 were selected, two- dimensional ultrasonography was performed first, then the fetuses suspected of fetal malformations underwent four- dimensional ultrasonography, the coincidence rates of diagnosis of fetal malformations by simple two-dimensional ultrasonography and two-dimensional uhrasonography combined with four-dimensional ultrasonography were compared and analyzed according to induced abortion, delivery and further examination results in superior hospitals. Results: Among 3 225 fetuses born by 3 192 middle and late pregnant women, 123 fetuses were found with fetal malformations (158 deformities), after induced abortion, delivery and further examination results in superior hospitals, 104 fetuses ( 135 deformities) were confirmed, 19 fetuses with fetal malformations (23 deformities) were lost to follow-up. Among 135 de- formities, the coincidence rate of diagnosis by two-dimensional ultrasonography combined with four-dimensional ultrasonography was 91.1% (123 deformities), which was statistically significantly higher than that by simple two- dimensional ultrasonography (77.8%, 105 deformities) (P〈0. 05) . After follow-up, 72 surface deformities were confirmed, the coincidence rate of diagnosis by two-dimensional ul- trasonography combined with four-dimensional ultrasonography was 91.6% (66 deformities), which was statistically significantly higher thanthat by simple two - dimensional uhrasonography ( 73. 6%, 53 deformities) (P 〈 0.05 ) .
