摘 要:目的探讨血清胱抑素c(Cysc)与原发性高血压患者颈动脉粥样硬化的关系。方法选择原发性高血压患者(高血压组)102例与体检健康者(对照组)96例,分别检测分析两组血清CysC等生化指标;应用彩色多普勒超声检查仪测定两组颈动脉内膜中层厚度(IMT),再根据是否有颈动脉斑块将所有入选对象分为健康无颈动脉斑块组(73例)、高血压无颈动脉斑块组(35例)和高血压合并颈动脉斑块组(67例),比较分析三组血清Cys C、生化指标、年龄及其与颈动脉IMT的关系。结果高血压组血清CvsC和颈动脉IMT明显高于对照组(P<0.05)。高血压合并颈动脉斑块组血清CvsC明显高于无颈动脉斑块组(均P<0.05)。harson相关分析发现颈动脉IMT与血清Cys C、年龄显著相关(r:0.26~0.32,P<0.01)。多元逐步回归分析显示CvsC是颈动脉IMT的独立危险因素(B=0.48,P<0.01);在调整年龄进行偏相关分析显示CysC与IMT的相关性差异仍有统计学意义(r=0.20,P<0.01)。结论血清CydC与原发性高血压颈动脉.MT有一定的关系,可作为其预测指标之一。Objective Investigate the correlation between serum cystatin C( Cys C) and Carotid atherosclerosis in patients with essential hypertension. Methods 102 patients with essential hypertension( Hypertension Group) and 96 healthy subjects( Control Group) were selected. The cystatin C and related biochemical indicators were detected. Carotid intima-media thickness( IMT) was measured by Doppler ultrasonography. The serum Cys C,age and IMT of the two groups were compared. After that,hypertensive patients and the healthy subjects were divided into three groups according to weather or not having carotid plaque. Then the serum Cys C,biochemical indicators,age of three groups were compared. Finally,analysis of the relationship between these indicators of all selected subjects with carotid IMT was made. Results Serum cystatin C levels and carotid IMT in hypertensive group were higher than that of normal control group( P < 0. 05). Serum cystatin C level in the hypertensive group with carotid plaque was higher than that of the other two groups( P < 0. 05). The spearman correlation analysis showed that carotid IMT was positively related to serum cystatin C and age( r = 0. 258,P < 0. 01; r = 0. 323,P < 0. 01). The multiple linear regression analysis showed that cystatin C was one of the independent risk factors of carotid IMT( B = 0. 48,P< 0. 01). Partial correlation analysis after the adjustment of age showed that there was a significant association between serum cystatin C level and carotid IMT( r = 0. 198,P < 0. 01). Conclusions There is a close correlation between serum cystatin C and carotid IMT in patients with essential hypertension; serum cystatin C can serve as one of the predicators of carotid IMT.
