不同温度下3种血液制剂中FⅧ活性分析

摘　　要：目的了解新鲜冰冻血浆、亚甲蓝病毒灭活新鲜冰冻血浆和冷沉淀融化后在不同环境温度中FⅧ活性变化情况。方法随机选取储存于-20℃以下1年内的新鲜冰冻血浆、病毒灭活血浆和冷沉淀置于融浆机中融化后,分别于0、2、4、8和24 h检测3种血液制剂中FⅧ活性[检测标本分别置于(4±2)℃冰箱和20℃恒温水浴箱中保存]。结果融化后(0 h)3种血液制剂的FⅧ的活性分别为(125.8±5.6)、(75.4±3.5)和(58.4±4.2),其中新鲜冰冻血浆中FⅧ活性明显高于亚甲蓝病毒灭活新鲜冰冻血浆和冷沉淀(P<0.001);保存于(4±2)℃和20℃中的3种血液制剂FⅧ活性在24 h后分别为(76.3±5.1)、(39.1±2.9)、(35.1±3.7)和(60.3±4.1)、(35.2±3.0)、(31.7±4.6),其活性均明显下降(P<0.001)。结论 3种血液制剂中新鲜冰冻血浆中的FⅧ活性较高,且均随保存时间的延长而活性下降。建议在(4±2)℃保存融化后的血浆制剂,并尽快输注(最好在8 h内输注)。Objective To study the activity of coagulant factor Ⅷ in thawed fresh frozen plasma( FFP),fresh frozen plasma with inactivated methylene blue pathogen( MB-FFP) and cryoprecipitate( Cryo) at different room temperatures.Methods FFP,MB-FFP and Cryo( kept below-20℃ and stored for one year) were randomly selected and thawed in Plasma Thawing System. The activity of coagulant factor Ⅷ in three blood components was measured at 0h,2h,4h,8h and24 h,respectively( the blood samples were stored either in a(4 ± 2) ℃ refrigerator or kept in a 20℃ water bath). Results The activities of coagulant factor Ⅷ in FFP,MB-FFP and Cryo were(125. 8 ± 5. 6),(75. 4 ± 3. 5) and(58. 4 ± 4. 2),respectively at 0h. The activity of FⅧ in FFP was significantly higher than in MB-FFP and Cryo( P<0. 001). The thawed FFP,MB-FFP and Cryo were stored for 24 h either at(4 ± 2) ℃ or 20℃. The activity of coagulant factor Ⅷ under those conditions were(76. 3 ± 5. 1),(39. 1 ± 2. 9),(35. 1 ± 3. 7).(60. 3 ± 4. 1),(35. 2 ± 3. 0),and(31. 7 ± 4. 6),respectively. The activities demonstrated a significant decline( P<0. 001). Conclusion The activity of coagulant factor Ⅷ in thawed FFP was higher than in MB-FFP and Cryo. Decline was most significant when the preservation time increased. Thus,the thawed FFP,MB-FFP and Cryo should be stored at a temperature of(4 ± 2) ℃ and be used in transfusion as early as possible( post-thaw 8h).
