海南地区无偿献血者不规则抗体筛查结果分析

摘　　要：目的调查分析本地区无偿献血者不规则抗体的发生频率和分布特点。方法随机抽取本中心2010年1月-2014年6月,无偿献血者血液标本344 148例,应用盐水法、凝聚胺法和抗球蛋白法筛查献血者不规则抗体。结果 344 148名无偿献血者中共检出不规则抗体78例,阳性率0.0227%。女性不规则抗体的检出率(0.0431%)明显高于男性(0.0129%)(P<0.05)。抗体阳性献血者,以18-24岁年龄段人群比率最高,达到47.44%。检出的不规则抗体包括ABO亚型16例,抗-M 9例,抗-D 3例,抗-P11例,抗-Tja1例,冷自身抗体20例和非特异性不规则抗体28例。结论海南地区无偿献血人群中有一定比例的红细胞不规则抗体检出率,因此加强和重视不规则抗体筛查和鉴定工作,对保障临床安全用血具有重要意义。Objective To study the frequency and distribution characteristics of irregular erythrocyte antibodies among volunteer blood donors in Hainan. Methods 344,148 blood samples of donors were randomly collected from January 2010 to June 2014. Irregular antibodies screening test was performed by saline agglutination test,polybrene test and anti-human globulin test. Results In total,data from 344,148 blood donors were analyzed,from which,78 irregular erythrocyte antibodies were detected(0. 0227%). The statistical results showed a higher frequency of irregular erythrocyte antibodies in females(0. 0431%) than in males( 0. 0129%)( P<0. 05). The highest irregular erythrocyte antibodies positive rate(47. 44%) was observed in the age group of 18-24 years. Among 78 positive samples with irregular erythrocyte antibodies were 16 cases of ABO subgroup,9 cases of anti-M,3 cases of anti-D,1 case of anti-P1,1 case of anti-Tja,20 cases of cold auto-antibodies and 28 cases of nonspecific irregular antibodies. Conclusion There was a certain rate of irregular erythrocyte antibodies among volunteer blood donors in Hainan area. Therefore,it is of great significance to strengthen the safety of clinical transfusion and to put an emphasis on screening and identification of irregular erythrocyte antibodies among volunteer donors.
