70岁以上老年患者行bio-Bentall手术的中期疗效分析

摘　　要：目的:评价在70岁以上老年患者中应用自制生物瓣带瓣复合管道行bio-Bentall手术的安全性。方法:回顾2006年3月—2014年6月23例行主动脉根部替换手术的70岁以上患者的临床资料,比较应用自制生物瓣带瓣复合管道与市售的机械瓣带瓣管道的手术疗效。结果:23例患者中15例应用生物瓣带瓣复合管道(生物瓣组),8例应用机械瓣带瓣管道(机械瓣组)。23例患者全部获得随访,平均随访时间(36.04±35.01)个月。两组均无院内死亡病例,两组术后并发症发生率差异无统计学意义。结论:70岁以上患者应用自制生物瓣带瓣复合管道行bio-Bentall手术与应用市售的机械瓣带瓣管道行Bentall手术的近中期疗效相近,自行缝制支架生物瓣人工血管复合管道的可行性和安全性较好,是行生物瓣主动脉根部置换的较理想的选择。Objective：To assess the safety of bio‐Bentall procedure with a handsewn composite bioprosthetic graft in the patients over the age of 70 years .Methods：From March 2006 to June 2014 ,23 patients over 70 years underwent aortic root replacement （Bentall procedure） with either a mechanical or a biological valve conduit .Early and midterm results were analyzed in patients undergoing aortic root replacement with two different kinds of grafts .Results：All of the 23 patients with either a mechanical （n=8） or a biological （n= 15） valve conduit were followed‐up .Averaged follow‐up time was （36 .04 ± 35 .01） months .There was no in‐hospital mortality ,and the incidence of postoperative complications were not significantly different between the two groups .Conclusions：This study suggests that the early and midterm results were not significantly different between aortic root replacement with either a mechanical or a biological valve conduit .Handsewn composite bioprosthetic graft is feasible and safe and can therefore be recommended for patients over the age of 70 years .
