柴胡疏肝散联合西药治疗肝硬化腹水随机平行对照研究

摘　　要：[目的]观察柴胡疏肝散联合西药治疗肝硬化腹水疗效。[方法]使用随机平行对照方法,将70例门诊及住院患者按病志号抽签法简单随机分为两组。对照组35例常规治疗,限制水钠、口服保肝类及利尿药、抗感染等常规西医治疗。螺内酯,40mg/次,3次/d;呋塞米,40mg/次,3次/d。治疗组35例柴胡疏肝散(茯苓、泽泻、丹参、大腹皮、枳实、白茅根、白术、茵陈、金钱草、猪苓等各30g,车前子、桂枝、桃仁等各15g),1剂/d,水煎200m L,1次/d;西药治疗同对照组。连续治疗60d为1疗程。观测临床症状、谷氨酸基转氨酶、白蛋白、谷草转氨酶、不良反应。治疗1疗程,判定疗效。[结果]治疗组显效20例,有效13例,无效2例,总有效率94.29%。对照组显效14例,有效12例,无效9例,总有效率74.29%。治疗组疗效优于对照组(P<0.05)。症状、体征积分两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。[结论]柴胡疏肝散联合西药治疗肝硬化腹水效果显著,值得推广。[Objective] To observe CHSGS liver cirrhosis combined with western medicine treatment efficacy. [Methods]Using random parallel control method, 70 cases of outpatient and inpatient numbers lottery law by disease Chi simple randomized into two groups. Conventional treatment group 35 cases, limiting sodium and water, oral liver class and diuretics, antibiotics and other conventional Western medicine. Spironolactone, 40 mg / time, 3 times / d; furosemide, 40 mg / times, 3 times / d. Treatment group 35 cases CHSGS（Fuling, Zexie, Danshen, Dafupi, Zhishi, Baimaogen, Baizhu, Yinchen, Jinqancao,Zhuling 30 g, Cheqianzi,Guzhi,Taoren each 15g）, 1 / d, decoction 200 m L, 1 times / d; western medicine with the control group. 60 d is a continuous course of treatment. Observation of clinical symptoms, acid base aminotransferase, albumin, aspartate aminotransferase, adverse reactions. A course of treatment to determine efficacy. [Results]Treatment group, 20 cases markedly effective in 12 cases, three cases, the total efficiency of 91.43%. Control group, 14 cases markedly effective in 13 cases, 8 cases, the total efficiency of 77.14%. Treatment group than the control group（P〈0.05）. Symptoms and signs were improved integration groups（P〈0.01）, improvement in the treatment group than the control group（P〈0.01）. [Conclusion]CHSGS combined western medicine liver cirrhosis significant effect, worthy of promotion.
